摘要
目的应用超声心动图的定量组织速度成像(QTVI)技术评价急性心肌梗死(AM I)患者急诊行经皮冠状动脉介入(PC I)术治疗前后左室局部收缩功能的变化。方法首次确诊为AM I的患者30例,且住院期间均行急诊PC I术,均行常规超声心动图检查,并应用QTVI技术测定各室壁二尖瓣环、基底段及中段3个位点在纵轴上的运动速度峰值(Sa),术后1、3个月随访资料完整,同时测量30例正常人相同室壁节段的相同参数作为对照。结果与正常对照组比较,AM I组术前各室壁均有明显减低(P<0.05);PC I术后1个月与术前相比较,前间隔中段、前壁中段、下壁基底段及中段Sa提高(P<0.05);术后3个月与术前相比较,除了侧壁中段Sa未见提高,其余室壁均有不同程度的提高(P<0.05);术后3个月与术后1个月相比较,除侧壁中段、前间壁中段、正后壁的基底段和中段未见提高,其余室壁Sa均有不同程度的增高(P<0.05);术后3个月时,后间隔中段、正后壁中段、下壁二尖瓣环及中段、前间隔的基底段和中段以及前壁Sa,与正常组比较差异无统计学意义(P>0.05),其余室壁局部心肌收缩功能仍较低(P<0.05)。结论 QTVI作为一项定量分析室壁运动的技术,是评价PC I疗效的重要手段之一。术后3个月时,梗死相关血管供血区域室壁局部收缩功能得到恢复。
Objective To evaluate the changes of left ventricular local systolic function in patients with acute myocardial infarction(AMI) before and after emergency PCI by quantitative tissue velocity imaging(QTVI) of Echocardiography.Methods There were 30 cases of first diagnosed with AMI during hospitalization underwent emergency PCI,each segment of each wall was measured velocity spectrum(Sa) by QTVI.Each case was followed-up after the first month and the third month.30 normal subjects were as the control group was measured in the same way.Results The control group compared with preoperative group,Sa of each segment of each wall reduced obviously(P0.01);AMI after PCI 1 month compared with preoperative group,the velocity of anterior septum(AS) in midst,anterior wall(AW) in midst,inferior wall(IW) in base and midst increased(P0.05);AMI after PCI 3months compared with preoperative group,except for the lateral wall(LW) in midst,the other wall movement velocity increased in varying degrees(P0.05);AMI after PCI 3months compared with 1 months,except for LW in midst,AS in midst and posterior wall(PW) in base and midst,the others increased in varying degrees(P0.05),after PCI 3months compared with the control group,there were no significant changes(P0.05) in the velocity of PW in midst,AS in base and midst,PW in midst,IW in mitral annular and midst,AW the other wall movement velocity still reduced in varying degrees(P0.05).ConclusionBeing a technique of measuring movement velocity of ventricular wall,it is the one of most important tools on evaluating the treatment effect after PCI.AMI after PCI 3months,the infarct-related artery blood supply to the regional wall systolic function was recovered.
