摘要
目的:了解肿瘤坏死因子-α、白介素-6在慢性阻塞性肺疾病(COPD)患者营养不良发生过程中的作用及与低氧血症的关系。方法:研究对象为45例COPD稳定期患者,将其分为Ⅰ组(营养不良组)24例、Ⅱ组(非营养不良组)21例,另设对照组25例;测定体重指数(BMI)、理想体重百分比(NW%)、体脂百分比(FAT%)、上臂中部臂围(MAC)、血清白蛋白(ALB)、总淋巴细胞计数(LYM);对COPD患者加测动脉血气分析;用放射免疫法测定45例COPD患者及对照组血清肿瘤坏死因子(TNF-α)、白介素-6(IL-6)浓度;分析TNF-α及IL-6与各营养参数及动脉血氧分压之间的相关性。结果:COPD组BMI、NW、MAC均比对照组低,TNF、IL-6均比对照组高,差异有统计学意义(P<0.05);Ⅰ组BMI、NW、FAT%、MAC、ALB均低于Ⅱ组及对照组,差异有统计学意义(P<0.05);Ⅰ组TNF-α与BMI呈显著负相关(r=-0.559,P<0.01);IL-6与Ⅰ组、Ⅱ组的PO2呈显著负相关(r=-0.699、-0.659,P<0.01)。结论:COPD患者血清TNF-α的浓度异常可能在COPD患者的营养不良发生中起到了一定作用;IL-6的水平与COPD患者的低氧血症呈明显相关性,可能与IL-6参与炎性反应、最终引起COPD患者的气道结构改变、影响其通气功能有关。
Objective:To investigate the roles of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6) in malnutrition and hypoxemia in patients with chronic obstructive pulmonary disease(COPD).Methods: Forty five patients with stable COPD and 25 controls were recruited.The patients were divided into malnutrition group and non-malnutrition group,which were include 24 and 21 cases,respectively.The body mass index(BMI),the percentage of ideal body weight(NW%),body fat percentage(FAT%),mid-upper arm circumference(MAC),serum albumin(ALB),total lymphocyte count(LYM),and serum levels of TNF-α and IL-6 were measured.Moreover,arterial blood gas analysis was performed in all cases.Then the data were analyzed to determine the correlation of TNF-α,IL-6,and various nutritional parameters with arterial partial pressure of oxygen.Results: BMI,NW,and MAC in cases were significantly lower than in controls,while the levels of TNF-α and IL-6 in cases were significantly higher than in controls.BMI,NW,FAT%,MAC,ALB in malnutrition group were significantly lower than in non-malnutrition group and control group.A negative correlation was found between the level of TNF-α and BMI in patients with malnutrition.And a negative correlation was also seen between IL-6 and PO2 in all cases.Conclusion: The serum TNF-α may be involved in malnutrition in patients with COPD.IL-6 is associated with hypoxemia in patients with COPD.
