深低温停循环在心血管手术中的应用 被引量:1

摘要 目的 回顾总结在心血管手术中应用深低温停循环技术的体外循环管理.方法 选取2006~2010年心血管病患者21例,标准全身麻醉,通过体表和全身体外循环血流降温,鼻咽温降至16~18 ℃,肛温降至18~20 ℃时,停止体外循环,其中5例采用上腔静脉逆行灌注,7例采用左颈总动脉顺行脑灌注.结果 全程转流时间83~305 min,阻断升主动脉时间37~240 min,深低温停循环时间4~99 min,上腔静脉逆行灌注15~57 min,左颈总动脉灌注10~82 min.本组手术4例因急性心功能衰竭死亡,2例因凝血功能障碍死亡,1例因肺动脉高压危象死亡,其余14例无神经系统并发症,康复出院.结论 深低温停循环技术能在复杂心血管手术中提供有效的脑保护,取得较为满意的临床效果.
