PFN-A治疗股骨粗隆间骨折147例疗效分析 被引量:15

摘要 目的探讨股骨近端髓内钉PFN-A(proximal femoral nail anti-rotation)治疗股骨粗隆间骨折的临床疗效。方法 2004年3月-2008年6月收住147例,按Evans分型,Ⅱ型26例,Ⅲ型66例,Ⅳ型39例,Ⅴ型16例。应用PFN-A内固定治疗。结果 126例获得随访,随访时间6-36个月,平均随访16个月。按Sanders髋关节创伤后功能评价方法,优72例,良31例,优良率81.75%。术后并发症:股骨头坏死3例,肺炎1例,术后2月部分负重后股骨颈锁钉有4mm向上切割1例,轻度髋内翻2例,切口浅部感染1例。结论 PFN-A治疗股骨粗隆间骨折,具有损伤小,手术时间短,出血少,固定牢固能早期下地活动,并发症少等优点,是治疗股骨粗隆间骨折的良好选择。 Objective To assess the curative effect of proximal femoral nail anti-rotation(PFN-A) for peritrochanteric fractures.Methods From March 2004 to June 2008,147 adult patients with peritrochanteric fractures were treated by PFN-A.According to Evans classification system,there were 26 cases in type Ⅱ,66 cases in type Ⅲ,39 cases in type Ⅳ and 16 cases in type Ⅴ.The follow-up period was between 6 to 36 months.Results According to Sanders scales,114 patients were in excellent,30 in good,and 3 in poor.Conclusion Peritrochanteric fracture treated by PFN-A has excellent clinical outcome.
