摘要
目的探讨肝脏空肠U型缝合技术在肝门部胆管癌切除后胆道重建中的应用价值。方法2006年11月～2010年10月施行肝门部胆管癌根治手术病人35例，根据入院时间和胆道重建方式，分为传统胆肠吻合组（A组）和肝脏空肠U型缝合组（B组），回顾性分析其临床资料。结果采用传统方式吻合者17例，U型缝合技术吻合者18例，术后暂时性胆漏的发生率分别为35．29％（6／17）和5。56％（1／18），两组比较，差异有统计学意义（P=0．0408）；吻合口不同程度狭窄的发生率分别为23．53％（4／17）和0（0／18），差异有统计学意义（P=0．0455）；手术时间A组（240±18）min和B组（220±12）rain，差异有统计学意义（P=0．0005）。结论采用肝脏空肠U型缝合技术进行肝71部胆管癌切除后的胆道重建，不仅简单易行、省时，而且术后胆漏、吻合口狭窄发生率低。
Objective To evaluate the value of surgical technique of ＂hepatieojejunostomy with circle discontinuous U suture＇ in the treatment for biliary reconstruction in radical resection of hilar cholangiocarc.inoma. Methods The clinical data of 35 patients with hilar cholangiocarcinoma surgically treated in our department between Nov. 2006 and Oct. 2010 were retrospectively analyzed. The patients were di- vided into 2 groups according to their admission date and mathods of bilia,＇y reconstruction,thai were group A（ traditional method） and group B（ cirele discontinuous U suture）. Results The rate of temporary bile leakage ＇after operation in group A was 35.29% （ 6/17 ） and 5.56% （ 1/I 8 ） in group, and the differenee was statistical （ P = 0. 0408） ; the rate of anastomotic stenosis was 23.53% （ 4/17 ） and 0 （ 0/18 ） respec- tively with statistical difference between the two groups （ P = 0. 0455 ）. Meanwhile the average operation time were （ 240 ± 18 ） minutes and （220 ± 12）minutes,and the difference was significant（ P =0. 0005）. Conclusion The surgical technique of ＂ hepaticojejunostomy with circle discontinuous U suture＂ in the treatment for hilar cholangiocareiuoma is a convenient and time-saving procedure, moreover,it can obviously decrease the incidence of temporary bile leakage anti anastomotic stenosis after operation.
