摘要
目的探索首诊医师负责制在慢性病患者健康管理中的效果,为其他地区的慢性病健康管理提供参考。方法将疾病诊疗中的"首诊医师负责制"原理引入到慢性病患者健康管理中的各个环节。通过现场问卷调查和日常工作记录收集方法,了解慢性病患者健康管理首诊医师负责制的实施效果。结果首诊医师负责制实施后,首诊测血压比例达到94.52%,筛查出的慢性病患者人数同比实施前增长228.76%,纳入管理人数同比增长169.23%,管理效果也明显改善。结论首诊医师负责制是一种有效的慢性病患者健康管理方式,能够促进社区慢性病健康管理工作的落实。
Objective To investigate the efficacy of the responsibility system of the first attending doctor in health management of patients with chronic diseases so as to provide a reference for other areas.Methods The principle of responsibility system of the first attending doctor in diagnosis and treatment of diseases was introduced to all aspects of health management of patients with chronic diseases.The field questionnaire survey and daily record of work were used to understand the efficacy of implementation of the responsibility system.Results After the implementation of the responsibility system of the first attending doctor,for the first visit of patient the ratio of measuring the blood pressure was 94.52%,and the percentage of the number screening patients with chronic diseases increased by 228.76%,the percentage of the patients′ number put into management increased by 169.23%,and the management effect was also improved.Conclusion The responsibility system of the first attending doctor is an efficacious method to health management of patients with chronic diseases,it can promote the implementation of the health management of chronic diseases in community.
