摘要
目的:分析探讨钉棒系统内固定在胸腰椎爆裂性骨折治疗中的临床疗效。方法:收集2007年至2010年以来在我院诊治的胸腰椎爆裂性骨折患者31例,患者均经进行核磁共振显影、术前X线、CT/MR I等临床影像学检测进行病症诊断。在C型臂X线机透视下,采用脊柱后路正中入路椎管探查减压,进行钉棒系统内固定术。术后进行随访,考察椎体高度、Cobb角及脊髓神经功能的恢复程度。结果:术后随访31例患者病,病椎椎体前缘平均高度由术前35.7%恢复到91.2%,呈显著性差异,具有统计学意义(P〈0.05);病椎椎体后缘平均高度由术前74.3%恢复到97.2%,呈显著性差异(P〈0.05);治疗前后Cobb平均角度由术前26.3°(19.1°-42.7°)恢复到4.6°(1.7°-7.3°),呈显著性差异(P〈0.05)。脊髓功能的损伤恢复程度使用Frankel评分法进行评价,仅2例无功能性改善,术后优良率达74.2%,好转率为19.3%。结论:钉棒系统内固定法安全可靠,操作方便,在胸腰椎爆裂性骨折治疗中疗效显著,能有效改善患者术后的生存质量。
Objective:To analyse the nail stick system discussed fixation in thoracolumbar burst fracture in the therapy of clinical curative effect.Methods:We collected in 2007 by 2010 in our hospital since diagnosis of thoracolumbar burst fracture patients(31 cases),patients were both by nuclear magnetic resonance imaging,preoperative X-ray,CT/MRI etc clinical imaging detection disease diagnosis.In C arm X-ray machine perspective,the posterior midline approaches intradiscal spine,probe decompression nail bar system fixation.Postoperative follow-up investigation corpectomy height,Cobb Angle and spinal neurological recovery degree.Results:The follow-up of 31 patients with vertebral disease vertebral disease,bottom margin of average height by preoperative 35.7% restored to 91.2% ft percentage,show significant difference are statistically significant(P0.05).Disease vertebral vertebral edge by the average height 74.3% restored to capsaicinoid is 97.2% preoperatively,show significant difference(P0.05).Cobb average Angle before and after the treatment by preoperative 2.63°(19.1°~42.7°)recovered to 4.6°~170°(7.3°),show significant difference(P0.05).Spinal cord injury recovery degree function use Frankel pointrating method to evaluate,only 2 cases of no functional improvement,postoperative recovery rate of 74.2%,19.3%in all the patients.Conclusion:Nail bar system internal fixation method is safe,reliable and convenient operation,in thoracolumbar burst fracture treatment curative effect is distinct,can effectively improve the life quality of patients who underwent the.
