摘要
目的总结十二指肠损伤18例的诊治体会。方法回顾性分析18例十二指肠损伤的临床资料。结果 18例患者均行手术治疗,5例为术前诊断(27.8%),术中诊断13例(72.2%)。治愈16例(88.9%),死亡2例(11.1%),多脏器功能衰竭死亡1例(5.6%),感染性休克死亡1例(5.6%)。结论十二指肠损伤术前诊断困难;尽早实施手术,选择恰当的损伤修复方式、缩短手术时间、有效的十二指肠减压和充分的引流是治疗的关键。
Objective To review the diagnosis and treatment of duodenal injuries in 18 patients.MethodsEighteen patients with duodenal injuries were retrospectively analyzed.ResultsAll the eighteen patients underwent surgical treatment.The preoperative diagnosis was available in five patients（27.8%） whilst intraoperative diagnosis was in 13 patients（72.2%）.The injuries healed in 16 patients（88.9%） whereas mortality occurred in two patients（11.1%）,including one due to multiple organ dysfunction syndrome（5.6%） and one due to septic shock（5.6%）.ConclusionPreoperative diagnosis of duodenal injury is challenging.The critical treatments include early surgical intervention,appropriate repair,short procedure,,and effective duodenal decompression and drainage.
