冠心病合并惊恐障碍患者自主神经功能的研究 被引量:6

A study on autonomic nerve function of coronary heart disease patients with panic disorder

摘要 目的通过监测惊恐障碍、冠心病及其共病时患者的心率变异性(HRV)变化来探讨惊恐障碍对冠心病患者自主神经功能的影响。方法所有入选者需符合冠心病或(和)惊恐障碍诊断及剔除标准,将入选者分为4组:冠心病组40例,惊恐障碍组36例,冠心病合并惊恐障碍组27例,健康对照组40例,采用24h动态心电图监测系统,观察HRV的时域、频域指标,采用方差分析,使用统计软件进行分析。结果与正常组相比,冠心病组、惊恐障碍组、冠心病合并惊恐障碍组的HRV各指标均降低,其中冠心病合并惊恐障碍患者HRV降低最明显,其时域指标SDNN、SDANN、PNN50、RMSSD分别为(70.40±14.74)ms、(91.72±24.46)ms、(2.83±2.07)%、(15.66±7.45)ms,频域指标LF、HF为(647.54±129.24)ms^2、(596.16±127.66)ms^2,与其余3组比较差异有统计学意义(P〈0.05)。结论冠心病、惊恐障碍患者自主神经功能紊乱,冠心病合并惊恐障碍患者自主神经功能紊乱更明显。 Objective To investigate the changes of autonomic nerve function of coronary heart disease (CHD) patients with panic disorder(PD). Methods All the subjects who met with the diagnostic code of CHD and PD were divided into CHD group(n=40) ,PD group(n=36) ,comorbid CHD and PD group(n=27) ,and 40 physical examinee were recruited as normal control group. They had a 24 hours Hoher ECG monitoring by time and frequency domain analysis of heart rate variability. ANOVA analysis was utilized to statistic the collected data. Re- suits Compared with normal controls,the patients of others groups had every indexs of HRV were reduced. The indexs of HRV of comorbid CHD and PD were lower than the patients of CHD or PD group. The score of time domain SDNN(70.40 ± 14.74) ms,SDANN(91.72 ± 24.46) ms ,PNN50(2.83 ± 2.07 ) % , RMSSD( 15.66 ± 7.45 ) ms,frequency domain LF(647.54 ± 129.24) ms^2 ,HF(596. 16 ± 127.66) ms^2 in comorbid CHD and PD. There were significant differences with others groups (P 〈 0.05 ). Conclusion The autonomic nervous functional of the patients with CHD and PD were in disorder. The autonomic nervous functional disorder of the patients with comor- bid CHD and PD was more severe.
