摘要
近年来,随着多项大规模临床试验的揭晓,生理性起搏的概念也发生了较大变化。在起搏模式上应用各种程控方式尽可能的减少右室起搏,鼓励自身传导。采用能尽量模拟心脏正常传导及最小的血流动力学改变的起搏部位。生物起搏器能更生理性的恢复心脏的起搏和传导功能,将是未来发展的方向。而远程监控、远程随访、病情监测等多种新功能提高了起搏器的工作效率,扩展了起搏器的功能。现就起搏模式和部位的生理化以及生物起搏器、起搏器的远程监控技术等方面的研究进展进行综述。
In recent years,the concept of physiological pacing has undergone great changes due to results obtained during several large clinical trials.With regards to pacing mode,many types of program control are used to reduce right ventricular pacing,and to promote self pacing.On the other hand,pacing positions are used which could mimic normal cardiac conduction and cause fewer hemodynamic changes.Building on from this,a biological pacemaker which could return normal cardiac pacing and conduction is the next step.This article discusses the current studies of physiological pacing mode and positions,the biological pacemaker and some new physiolgical function of pacemaker.
