The Value of Surgical Measurement of the Sphenoid Sinus with Sagittal CT

摘要 目的探讨CT矢状位重组图像测量蝶窦外科径线的价值。资料与方法在薄层CT矢状位重组图像上对173名成年健康自愿者(男91名,女82名)的343个蝶窦的部分径线进行测量。结果除外部分特殊病例(如Onodi气房)外,从蝶窦口至前颅窝底及蝶窦下壁最低点的垂直距离分别平均为(9.54±2.5)mm及(12.64±3.7)mm。从蝶窦口至蝶窦后壁的水平距离平均为(18.13±1.6)mm或(28.9±2.5)mm。从蝶鞍最低点向前至蝶窦前壁的水平距离平均为(17.72±1.5)mm。除部分蝶骨小翼过度气化的病例外,沿前颅窝底从蝶窦的前壁至其后壁的水平距离为(9.08±4.3)mm。除部分斜坡过度气化病例外,从蝶窦后壁最远点向前至蝶窦前壁的水平距离平均为(21.82±7.4)mm。结论 CT矢状位重组可用以测量蝶窦的解剖径线,有助于避免外科手术时并发症的发生,但对部分过度气化的蝶窦需个别对待。因此,术前蝶窦矢状位CT的观察至关重要。 Objective To discuss the value of the surgical measurement of sphenoid sinus with sagittal CT thin-slice reconstruction images.Materials and Methods 343 CT images of the sphenoid sinus from 173 healthy volunteers(91 males and 82 females) were measured with sagittal CT reconstruction images.Results Except for some special cases(such as Onodi cell),the mean vertical distance from the sphenoid ostium to the skull base,and to the lowest point of the bottom of the sphenoid sinus was 9.54±2.5 and (12.64±3.7) mm.The mean horizontal distance from sphenoid ostium to the nearest point of posterior wall of sphenoid sinus was (18.13±1.6)mm or (28.9±2.5)mm.The mean horizontal distance from the lowest point of sella to anterior wall of sphenoid sinus was (17.72±1.5)mm.Except for cases that the pneumatization extends into the lesser wing,the mean horizontal distance from anterior wall to posterior wall of sphenoid sinus lining skull base was (9.20±0.7)mm.Except for clival recess type sinus that pneumatizes inferiorly,the mean horizontal distance from the dorsal farest point of posterior wall of sphenoid sinus to anterior wall was (21.82±7.4)mm.Conclusion CT sagittal thin-slice reconstruction images provides anatomical information and the surgical distance about the sphenoid sinus that are essential for avoiding surgical complications,but variations in the extensions of pneumatization should be dealt with specially.
