摘要
目的：探讨选择性椎弓根置钉在多节段脊髓型颈椎病并颈椎失稳颈后路单开门手术中的应用。方法：我院2009—01-2010—04收治多节段脊髓型颈椎病并颈椎失稳37例,术前所有患者X线片测量均有椎体间不稳,CT示均有不同程度的椎间盘突出及骨赘形成,磁共振显示多节段脊髓受压明显。所有患者均采用颈后路单开门选择性椎弓根置钉内固定,根据具体情况,其中采用单侧椎弓根置钉16例,采用双侧椎弓根置钉21例。结果：术中未出现并发损伤,术后影像学检查提示置钉位置正确,短期内单双侧椎弓根置钉内固定均能达到牢靠固定,无松动及断钉现象,均于3~5个月获得良好的骨性愈合,脊髓神经功能有不同程度的恢复。结论：颈后路单开门选择性椎弓根置钉内固定是治疗脊髓型颈椎病并颈椎失稳的一种行之有效的方法。
Objective：To discuss the application of selective pedicle screw in posterior single open-door in treatment of cervical spondylotic myelopathy combined with cervical spine instability.Methods：Thiety seven patients with cervical spondylotic myelopathy combined with cervical spine instability from January 2009 to april 2010 were choosed.The preoperative radiographs of 37 patients were measured and were diagnosed to be cervical spondylotic myelopathy combined with cervical spine instability.CT shows varying degree slipped disk and the bone superfluous formation.MRI showed multi-stage spinalcords were compressed obviously.Posterior single open-door and selective pedicle screw were adopted in all 37 patients.Depending on special condition,16 cases of 37 patients adopted unilateral pedicle screw fixation,21 cases adopted bilateral pedicle screw fixation.Results：Complication were not found after the operation.The postoperative radiographs showed that the screws were in the right places.The short-term radiographs showed firmly fixed screws,and no breakage of plates and screws were found.Solid arthrodesis was achieved in 37 patients（100%） in 3~5 months.The spinal nerves function has the varying degree restoration.Conclusion：Posterior single open-door and selective pedicle screw is one effective method in treatment of cervical spondylotic myelopathy combined with cervical spine instability.
