摘要
目的对比分析"萤火虫"成像技术和二维高频超声显示甲状腺内细砂粒样钙化对甲状腺乳头状癌诊断的临床意义。方法选取我院甲状腺外科2010年1月~2010年7月128例甲状腺结节手术患者(共计160个结节),良性结节96个,恶性结节64个,其中甲状腺乳头状癌结节63个,甲状腺髓样癌结节1个。术前患者均进行甲状腺二维高频超声及"萤火虫"成像技术检查,重点观察甲状腺结节内有无细砂粒样钙化,诊断甲状腺乳头状癌,并与术后病理结果对照分析。结果二维高频超声检出甲状腺内细砂粒样钙化诊断甲状腺乳头状癌的敏感性54.0%(34/63)、特异性78.4%(76/97)、精确度68.8%(110/160);"萤火虫"成像技术诊断甲状腺癌的敏感性81.0%(51/63)、特异性81.4%(80/97)、精确度81.3%(130/160)。两者对比有显著差异性(P<0.05)。结论 "萤火虫"成像技术在检出细砂粒样钙化诊断甲状腺乳头状癌的敏感性、特异性、精确度均高于二维高频超声。
Objective A comparative study between firefly and 2-D high-frequency ultrasound imaging technology in thyroid microcalcification diagnosis of papillary thyroid cancer.Methods Retrospectively analyze clinical data of 128 thyroid disease patients in our hospital from January to July in 2010,of which 96 cases were malignant and 64 cases were benign.In all these cases,63 cases were diagnosed papillary thyroid cancer.They were diagnosed initially by 2-D high-frequency ultrasound and firefly before surgical operations.Compared with pathological examinations,papillary thyroid cancer was diagnosed by finding microcalcification in thyroid node with firefly technology and 2-D high-frequency ultrasound.Results The sensitivity,specificity and accuracy of calcification intra-thyroid by 2-D high-frequency ultrasound diagnosis of papillary thyroid cancer was 58.7%,81.4% and 72.5% respectively.The sensitivity,specificity and accuracy of microcalcification intra-thyroid by firefly diagnosis of papillary thyroid cancer was 92.1%,82.4% and 86.3% respectively.Compared with 2-D high-frequency ultrasound,firefly image had higher sensitivety,specificity and accuracy(P0.05).Conclusion Firefly is better than 2-D high-frequency ultrasound for the diagnosis of papillary thyroid cancer by finding microcalcification.
