摘要
目的探索社区组织(NGOs)如何与疾病预防控制机构(CDC)、医疗机构更加紧密地合作,建立具有实效的、符合当地文化和经济背景的艾滋病病毒(HIV)感染者/病人综合管理模式。方法分析项目实施策略、过程和产出,对相关合作机构和受益人群进行焦点小组访谈和个案访谈。结果项目的实施,建立了昆明市"三位一体"HIV感染者/病人综合管理模式,充分体现政府主导,多部门合作的工作原则,也为NGO充分参与艾滋病防治工作提供了必要的支持性环境和资金保障。正确引导HIV感染者进行治疗,并配合医院开展依从性教育、提供心理支持。促进了HIV感染者/病人随访关怀工作及转介机制,感染者和病人的CD4检测及抗病毒治疗转介成功率达100%,对发现新报告感染者告知率达到100%。结论随着艾滋病防治工作的拓展,NGO小组在各地区参与感染者/病人的随访关怀及提供转介服务的重要性不断显现,但CDC、抗病毒治疗医疗机构、NGOs的职责、人员能力以及协调机制的建立和完善仍然是有效工作模式建立的重要影响因素。
Objective To explore how NGOs best collaborate with CDCs and hospitals in Kunming so as to construct a comprehensive management model for PLWHA tailored to local cultural and economic conditions. Methods Strategies, process and outcomes of the project implementation were analyzed; focus group discussions and indepth interviews were conducted among target population and stakeholders; and the experience learnt from the project to improve current care system was summarized. Results Project implementation promoted care, follow up and referral of PLWHA. The rate of testing HIV carriers/ AIDS patients and referring them to antiretroviral therapy reached 100 %, and the rate of notifying newly detected HIV cases was also 100%. Conclusion With the expanding of AIDS control and prevention, NGOs are increasingly involved in PLWHA＇s care, follow up and referral service, However, clear responsibilities of CDCs, hospitals and NGOs , capacity of participating staffs and building of an appropriate coordination mechanism still play an important part in constructing an effective management model for PLWHA.
