机体应激强度与环核苷酸系统的动态变化 被引量:5

Perioperative dynamics of cyclic ribonucleotide in patients under stress state during cardiac surgery

摘要 目的 为探讨环核苷酸系统在创伤应激状态下作为机体内分泌系统第二信使的变化及意义。方法 以放免法测定 16例低温麻醉体外循环者围术期 7个时段的血浆cAMP、cGMP值。结果 发现随机体承受刺激强度的增减 ,cAMP呈同步改变 ,而cGMP增高主要与心脏承受手术操作和体外循环刺激有关。结论 认为对血浆环核苷酸系统的动态观察可作为机体创伤应激反应强度的一个综合参考指标 ,体外循环心脏直视手术亦可作为特定的机体创伤模式为科研及临床观察所用。 Aim:To investigate the dynamics of plasma second messenger of endocrine system during the perioperative period of cardiac surgery,and its relationship to traumatic stress.Methods:Sixteen patients,aged 19 31±10 4 years undergoing open heart surgery with CPB and hypothermia,served as subjects.The arterial plasma concentrations of cAMP and cGMP were measured dynamically by radioimmunoassay during the perioperative period.Preoperative blood samples of the patients were heparinized and were taken as heparin controls and the venous blood samples of 30 healthy blood donors as normal controls.Results:There were no statistical differece among heparin control,preoperative level and normal control.The peak values occurred during CPB,and plasma cAMP levels changed synchronously with the intensities of operative stimuli to human body,however cGMP levels was mainly related to operative stimuli to heart and CPB.Conclusion:It is suggested that dynamic variation of plasma cyclic ribonucleotide could be considered as a reference parameter for the intensity of traumatic stress and the open heart surgery with CPB could be used as a special traumatic model of human for scientific research.
