摘要
目的:探讨缺血后处理对大鼠肝移植缺血再灌注损伤的作用.方法:SD大鼠随机分成6组.(1)假手术组:开腹后仅游离肝周韧带;(2)缺血再灌注组:仅行大鼠原位肝移植术;(3)缺血后处理10s/10s组:大鼠原位肝移植术后门静脉开放前行再灌注10s/阻断10s的缺血后处理;(4)缺血后处理30s/30s组:后处理为再灌注30s/阻断30s;(5)缺血后处理60s/60s组:后处理为再灌注60s/阻断60s;(6)缺血后处理90s/90s组:后处理为再灌注90s/阻断90s.后处理组再灌注/阻断6个循环后完全开放门静脉.分别于门静脉恢复血流后2h和6h采集标本,观察血清肝功能、肝组织病理改变、肝脏组织丙二醛(MDA)和超氧化物歧化酶(SOD)的含量,此外,还观察各组大鼠的存活率.结果:缺血后处理30s/30s和60s/60s组在术后2h和6h,与缺血再灌注组相比肝功能明显改善,肝脏组织病理形态学损伤明显减轻,其中缺血后处理60s/60s组效果最佳,其余后处理组对肝脏缺血再灌注损伤没有保护作用.缺血后处理60s/60s组与缺血再灌注组相比,术后6h肝组织MDA水平明显降低(8.69μmol/g±1.68μmol/g vs14.31μmol/g±2.43μmol/g,P=0.0087),而SOD活性则显著增高(10.01UN/mg±0.89UN/mg vs6.92UN/mg±1.19UN/mg,P=0.0092).除假手术组外,缺血后处理60s/60s组大鼠存活率明显高于其他组.结论:缺血后处理能减轻大鼠肝移植缺血再灌注损伤,减轻肝组织氧化应激,提高大鼠存活率,其中以再灌注60s/阻断60s循环作用最佳.
AIM:To explore whether ischemic postcondi-tioning protects against ischemia/reperfusion injury in rats undergoing liver transplantation.METHODS:Male Sprague-Dawley rats were divided randomly into six groups:animals un-dergoing sham operation(sham),ischemia/re-perfusion(I/R),and ischemic postconditioning consisting of 6 cycles of 10 s/10 s(post-10 s),30 s/30 s(post-30 s),60 s/60 s(post-60 s)or 90 s/90 s(post-90 s)reperfusion/reocclusion.At 2 and 6 h after portal vein reperfusion,blood and liver tissue samples were taken to determine liver function,histopathology,and liver homogenate MDA and SOD levels.Moreover,the survival rate of rats was measured.RESULTS:At 2 and 6 h after reperfusion,post-30 s and post-60 s reperfusion/reocclusion markedly improved liver function,ameliorated hepatocellular damage,lowered MDA level,and elevated SOD activity compared with the I/R group(8.69μmol/g±1.68μmol/g vs 14.31 μmol/g±2.43μmol/g,P=0.0087;10.01 UN/mg ±0.89 UN/mg vs 6.92 UN/mg±1.19 UN/mg,P=0.0092).In contrast,post-10 s and post-90 s reperfusion/reocclusion failed to protect against liver damage.Excluding the sham-operated group,rat survival rate was significantly higher in the post-60 s group than in the other groups.CONCLUSION:Ischemic postconditioning can reduce reperfusion injury,attenuate oxidative stress,and improve survival in rats undergoing liver transplantation.Ischemic postconditioning consisting of 6 cycles of 60 s/60 s reperfusion/reocclusion exhibits the best protective effects against ischemia/reperfusion injury in rats un-dergoing liver transplantation.
