急性胸主动脉夹层的外科治疗 被引量:2

摘要 目的总结急性胸主动脉夹层的外科治疗经验。方法 76例胸主动脉夹层患者术前均经超声心动图及胸部增强CT检查明确诊断,其中Stanford A型胸主动脉夹层41例,行Bentall手术29例,Wheat手术2例,升主动脉人工血管置换1例,深低温停循环(DHCA)下行升主动脉及主动脉弓替换(包括同期象鼻手术)9例。B型胸主动脉夹层35例均行腔内隔绝手术。结果 41例StandfordA型患者手术死亡3例(7.3%)1,例术后2个月死于抗凝过量致颅内出血,3例远端夹层复发。35例B型患者手术均获成功,1例术后死于消化道出血。结论急性胸主动脉夹层只有及时明确诊断,积极手术治疗,才能有效挽救患者的生命;主动脉弓替换时应加强脑保护,DHCA时选择性脑灌注效果好;B型夹层采用腔内隔绝术创伤小,效果佳。
