摘要
目的探讨下腔静脉滤器在治疗下肢深静脉血栓(DVT)中的应用价值。方法 66例下肢深静脉血栓患者治疗前置入下腔静脉滤器,其中30例采用抗凝、溶栓等药物+气压梯度治疗,另36例实施手术治疗,并同时采取抗凝、溶栓及气压梯度治疗。观察本组患者有无出现肺栓塞的症状及体症,定期透视滤器的形态与位置。结果下腔静脉滤器置入全部成功,经治疗下肢深静脉血栓症状及体征消失,无肺栓塞发生。随访2～24个月,滤器无移位,未发生肺栓塞。结论下腔静脉滤器置入可有效防止肺动脉栓塞的发生,可为治疗下肢深静脉血栓提供有效保障。
Objective To discuss and evaluate the value of insertion of inferior vena cava（IVC）fiher in treating lower extremity deep venous thrombosis（DVT）.Methods Inferior vena cava filters were placed in 66 patients with DVT prior treatment,30 patients with DVT were treated with anticoagulation,thrombolysis and pressure gradient;36 patients by operation, thrombolysis and pressure gradient.Whether patients occurred pulmonary embolism were observed clinically,and the form and site of filters were monitored by periodic fluoroscopy.Results Inferior vena cava filter were placed successfully in all patients. Symptoms and signs of DVT disappeared in all the patients after treatment. None of the patients occurred pulmonary embolism in this series.Follow up 2-24 months, the patients suffered neither from migration complication of the filter,nor from pulmonary embolism.Conclusion The method of inserting IVC filter is simple and safe,and can prevent pulmonary embolism effectually.h offers ensure for the treatment of DVT.
