摘要
目的 了解先天性心脏病患儿血浆降钙素基因相关肽(CGRP)及血浆内皮素(ET)的变化,探讨其在先天性心脏病继发的病理生理改变中的作用及其临床意义。方法 左向右分流型先天性心脏病患儿29 例,其中11例成功地行修补术,正常对照组25 例。用放射免疫均相竟争法分别检测血浆CGRP及ET水平。结果 先天性心脏病患儿术前血浆CGRP较对照组低(P< 0.01),而ET较对照组高(P< 0.01);患儿术后血浆CGRP较术前有增高趋势,但差异不显著(P> 0.05),而其ET 较术前低(P< 0.05);先天性心脏病患儿血浆CGRP与ET成直线负相关(r= - 0.71 P< 0.01)。结论 左向右分流型先天性心脏病患儿血浆CGRP的降低与ET的增高可能共同参与了肺动脉高压的形成。
Objective To study the clinical significance of plasma endothelin(ET)and calcitonin gene related peptide(CGRP)and their effect on the secondary change of pathophysiology in congenital heart disease(CHD). Methods There were 29 cases of children with CHD,11 cases of which were performed surgical repair successfully,and another 25 children as controls.The radioimmunoassary was used to examine their plasma ET and CGRP,respectively. Results The children with CHD before operation showed a significant increase in plasma ET level and a significant decrease in plasma CGRP level compared with controls,respectively(all P < 0.01 ).After successful surgical repair,the elevated plasma ET le vels were induced( P <0.05),and the plasma CGRP level was increased,but P >0.05.There was a negative correlation r =-0.71 P <0.01;between plasma ET and CGRP in children with CHD before surgical repair. Conclusion Increase of ET and decrease of CGRP may play an important role in causing pulmonary hypertension in CHD.
