腰椎间盘MRI高信号区在椎间盘源性下腰痛诊断中的意义 被引量:3

摘要 目的探讨腰椎间盘MRI高信号区(HIZ)现象与椎间盘源性下腰痛的相关性。方法选择2009年4月-2010年11月22例经保守治疗无效、CT影像上无腰椎间盘突出的下腰痛患者行腰椎MRI检查和腰椎间盘造影术,评估腰椎间盘高信号区现象在腰椎间盘源性下腰痛诊断中的价值。结果22例下腰痛患者中,有68个椎间盘行腰椎间盘造影术,结果有26个椎间盘出现疼痛复制反应,其中有12个椎间盘显示高信号区。12个高信号区的椎间盘在造影中均呈现出2-3级的纤维环破裂和疼痛复制反应。结论无腰椎间盘突出的下腰痛患者存腰椎间盘MRI上存在椎间盘内高信号,说明这个椎间盘是产生疼痛的破裂椎间盘,也表明HIZ可作为诊断椎间盘源性下腰痛的重要征象。 Objective To discuss the correlation of high-intensity zone ( HIZ ) of lumbar intervertebral disc on MRI and discogenic low back pain. Methods To choose 22 cases low back pain patients from April 2009 to November 2010, all the patients had treated with the expectant treatment but were of no effects , aud no lumbar disc protrusions on CT, and to be checked by MRI and lumbar discogTaphy.To evaluate the value of high-intensity zone ( HIZ ) of lumbar intervertebral disc on MRI and discogenic low back pain. Results 22 cases low back pain patients had 68 intervertebral disks would be done the lumbar discography ,26 intervertebral disks had reproduce pain reactions, and 12 interveriebral disks were showing high-intensity zone. The intervertebral disks in 12 high-intensity zones were all appearing 2 to 3 degree brnken annulus fibrosus and reproduce pain reactions .Conclusion The low back pain patients with no lumbar disc protrusions on MRI in diagnosing, there are intervertebral disks in high-intensity zone, to account for the intervertebral disk is to produce the painful broken annnlus fibrosus, and also to indicate the HIZ can be diagnose discogenic low back pain.
