摘要
目的探讨小儿过敏性紫癜的心肌酶谱变化及临床意义。方法选择过敏性紫癜患儿129例,根据入院时临床症状分为混合型和单纯型两组:混合型组81例(男42例,女39例),单纯型组48例(男21例,女27例)。两组患儿均于入院后次晨抽取空腹静脉血3 mL,采用全自动生化分析仪酶法进行心肌酶谱检测,分析两组患儿单项心肌酶谱升高人数、心肌酶谱检测结果及其临床意义。结果两组患儿中,血清心肌酶谱升高81例(62.79%),其中过敏性紫癜混合型组占51例(62.96%),单纯型组占30例(62.50%);α-羟丁酸脱氢酶升高人数最多,共66例(51.16%),其中过敏性紫癜混合型占39例(48.15%),单纯型占27例(56.25%);肌酸激酶同工酶(CK-MB)升高42例(32.56%),其中过敏性紫癜混合型占36例(44.44%),单纯型6例(12.50%),过敏性紫癜混合型患儿CK-MB升高人数较单纯型明显升高,二者比较差异有统计学意义(P<0.05);过敏性紫癜混合型患儿CK-MB检测结果升高明显,与单纯型比较差异有统计学意义(P<0.05)。结论过敏性紫癜患儿无论混合型还是单纯型均可有心肌酶的变化;过敏性紫癜患儿α-羟丁酸脱氢酶升高人数最多;过敏性紫癜混合型患儿心肌酶谱变化明显,主要以CK-MB最为敏感;过敏性紫癜混合型心脏受累的概率比单纯型要高(P<0.05)。
Objective To study the change and its clinical significance of the myocardial enzymes in children with Henoch-Schonlein purpura(HSP).Methods The data about myocardial enzymes in 129 children diagnosed with HSP were reviewed retrospectively.According to the clinical symptoms on admission,129 cases were divided into two groups: mixed purpura and simple purpura.The serum myocardial enzymes in the next morning after hospitalization were detected.The increasing number of single myocardial enzyme and the results of the myocardial enzymes were analyzed between the mixed purpura and the simple purpura groups.Results Among 129 cases,serum myocardial enzymes increased in 81 cases,containing 51 cases of the mixed purpura and 30 cases of the simple purpura.α-HBDH increased at the largest,which accounted for 51.16%(66 cases),containing 39 cases of mixed purpura and 27 cases of simple purpura.CK-MB increased in 42 cases,containing 36 cases of mixed purpura and 6 cases of simple purpura.In mixed purpura group,the increasing number of CK-MB was higher than that of simple purpura group(P0.05).There was a significant difference in the increasing level of CK-MB between mixed purpura and simple purpura groups(P0.05) and CK-MB increased more in mixed purpura group.Conclusion Henoch-Schonlein purpura can cause the changes of the myocardial enzymes.The number of children with α-HBDH increasing is the largest.In mixed purpura,the changes of the myocardial enzymes are greater than those in simple purpura,especially in CK-MB.In mixed purpura,the probability combined with cardiac damage is higher than that in simple purpura(P0.05).
