摘要
目的:了解乡村一体化制度下村医的薪酬与待遇现状。方法:使用定性调查,同时结合文献和政策文件,进行典型案例分析。结果:宁夏回族自治区和江苏省的乡村一体化管理模式各有特色。宁夏村医收入来源主要为政府补助和公共卫生经费;江苏省则以业务收入为主。在宁夏,乡镇卫生院对村卫生室的各项工作考核后发放政府补贴与公共卫生经费;江苏省村医的收入统一上交给乡镇卫生院管理,经考核后按照统一比例返还。两省村医收入水平在村里属于中等水平,大多数村医仍处于半农半医状态。两省村医都参加了当地的新型农村合作医疗;江苏省已解决村医的养老问题,而宁夏仍然没有解决。两省村医都以中老年为主,年轻人流出较多,流入不足,稳定性不高。讨论和建议:尽快明确村医身份,保障村医的合法权益;因地制宜,采取多种方法,提高村医收入;推进村医养老问题的解决,避免人才流失和村医空白。
Objective To understand the situation of compensation and welfare of village doctors under integrated management of township and village health facilities.Methods Interview and FGDs were conducted in combined with literature and policy document review to do a case study.Results The integrated management of township and village health facilities in Ningxia Hui Autonomous Region and Jiangsu province had different characters.The main income of village doctors in Ningxia was governmental subsidy and public health funding,while that in Jiangsu was mainly medical income. In Ningxia township hospitals were responsible for the assessment on village clinics'medical services and public health services.The results of assessments were related to village doctors'governmental subsidy and public health funding.In Jiangsu,all income of village doctors were at first handed in to township hospitals,then the profit was returned to village doctors according to the assessment results of health services.The income level of village doctors in Ningxia and Jiangsu were in the middle level among villagers,meanwhile,most village doctors needed to get financial support from farming.All village doctors in those two provinces had participated in the New Rural Cooperative Medical Scheme.However,village doctors in Jiangsu had pension insurance while those in Ningxia had not.Meanwhile,the majority of village doctors in both provinces were middle-aged and the lack of stability of village doctors was still a serious problem.Conclusion Village doctors'functions and characters in health system should be identified as soon as possible,to protect their legitimate interest.Various ways should be adopted to raise village doctors'income.The solution to village doctors'pension problem should be propelled to avoid the brain drain.
