摘要
【目的】评价高频振荡通气(high~frequency oscillatory ventilation,HFOV)治疗新生儿急性呼吸窘迫综合征(acute respiratory distress syndrome,ARDS)的疗效。【方法】对本院2009年1月至2010年2月30例患ARDS新生儿经常规机械通气(conventional mechanical ventilation,CMV)无效后改HFOV治疗,观察HFOV治疗前后肺功能变化及病情转归。【结果】26例(86.67%)治愈,无好转放弃治疗4例。治愈患儿中HFOV治疗2h后,动脉氧分压(PaO2)由HFOV治疗前的(45.54±9.31)mmHg上升为(78.36±12.81)mmHg(P〈0.05),吸氧体积分数(FiO2)由HFOV治疗前(0.60±0.14)降低为(0.49±0.15)(P〈0.05),PaO2/FiO2由(100.32±41.54)增加到(174.32±68.14)(P〈0.05);12h后,动脉二氧化碳分压(PaCO2)由(49.08±12.45)mmHg下降到(40.38±10.81)mmHg(P〈0.05)。【结论】HFOV用于新生儿ARDS疗效好,能较快改善肺氧合功能,减少并发症。
[Objective] To evaluate the efficacy of high-frequency oscillation ventilation(HFOV) for the treatment of neonates with acute respiratory distress syndrome(ARDS). [Methods] Thirty neonates with ARDS after failing to conventional mechanical ventilation(CMV) turned to receive HFOV treatment. The change of pulmonary function and the prognosis were observed before and after treatment. [Results] Twenty six patients(86.67%) were cured. After 2h treatment with HFOV, PaOe in these curative neonates increased from (45.54±9.31)mmHg to (78.36±12.81)mmHg( P 〈0.05), and FiO2 decreased from (0.60±0.14) to (0.49±0.15)( P 〈0.05), and PaO2/FiO2 increased from (100.32±441.54) to (174.32±68. 14)( P 〈0. 05). After 12h treatment with HFOV, PaCO2 decreased from (49.08±12.45)mmHg to (40.38± 10.81)mm- Hg( P 〈0.05). [Conclusion] HFOV is effective in treating the neonates with ARDS, and can improve the pulmonary function quickly and reduce the complications.
