输尿管镜下钬激光碎石与气压弹道碎石治疗输尿管结石的临床疗效比较 被引量:6

Clinical comparison of ureteroscopic holmium:YAG laser lithotripsy and pneumatic lithotripsy in the treatment of ureteral calculi

摘要 目的 比较输尿管镜下钬激光碎石术(holmium laser lithotripsy,HL)与气压弹道碎石术(pneumatic lithotripsy,PL)治疗输尿管结石的疗效和安全性.方法 回顾分析2007年6月至2010年1月采用输尿管镜技术治疗输尿管结石患者的临床资料,其中PL治疗223例、HL治疗214例.结果 手术总的碎石成功率HL为96.26%(206例),PL为88.34%(197例),两组间碎石总成功率差异无统计学意义;但是平均手术时间HL组为32.8min,PL组为42.5min,P〈0.05;平均结石排净时间HL组为17d,PL组为33d,P〈0.05;手术并发症发生率HL组(1.87%)少于PL组(13.9%),P〈0.05.结论 HL和PL均是治疗输尿管结石的有效方法,只要提高操作熟练程度并及时总结经验,就可提高这两种手术的成功率. Objective To compare the efficacy of holmium laser - assisted ureteroscopy and pneumatic lithotripsy in the treatment for ureteral calculi. Methods A total of 437 patients with ureteral calculi who underwent ureteroscopy between June 2007 and January 2010 in our hospital were enrolled in the study. Pneumatic lithotripsy was performed in 223 cases and holmium: YAG laser lithotripsy in 214. Results The overall success rate for the holmium:YAG laser lithotripsy (96.26%) was higher than that of pneumatic lithotripsy( 88.34% ), but the difference was insignificant ( P 〉 0.05 ). The operation time was much shorter in the holmium : YAG laser lithotfipsy group than in the pneumatic lithotripsy group (32.8 min vs 42.5 min,P 〈0. 05 ). The total time for eliminating ureterolith averaged 17 d and 33 d re- spectively in the holmium:YAG laser lithotripsy group and the pneumatic lithotripsy group with difference being significant ( P 〈 0.05 ). The complication rate was higher in the pneumatic lithotripsy group than in the holmium:YAG laser lithotripsy group( 13.9% versus 1.87%, P 〈 0.05 ). Conclusion Both uret- eroscopic holmium laser lithotripsy and pneumatic lithotripsy are effective in the treatment of ureteral cal- culi, and the indications for holmium laser lithotripsy are wide. With the operational proficiency improved and surgical experience accumulated, the success rate of these two operations will be enhanced.
