顺行灌注冷晶体停搏液及温血停搏液对心肌的保护作用 被引量:3

Studies on the myocardial protective effects of antegrade cold crystalloid and warm blood cardioplegia administration

摘要 目的研究冷晶体及温血停搏液对心肌的保护作用。方法将96只猫随机分为体外循环(CPB)组(Ⅰ)、损伤组(Ⅱ)、冷晶保护组(Ⅲ)及温血保护组(Ⅳ)。观测各组心功能、膜ATP酶活性和膜蛋白颗粒的变化。结果组Ⅰ各时间点各项指标无变化。缺血期间,组ⅡⅣNaKATPase活性下降(P<0.01);组Ⅱ、ⅢCaMgATPase活性下降,膜蛋白颗粒数减少(P<0.05),组Ⅳ无减少(P>0.05)。复灌后,组Ⅳ心功能、CaMgATPase活性恢复正常,NaKATPase活性和膜蛋白颗粒数恢复稍差。结论单纯CPB对心功能、膜蛋白无损伤作用;温血停搏液心肌保护作用较好,但仍需完善。 Objective To study the myocardial protective effects of cold crystalloid cardioplegia and warm blood cardioplegia administration. Methods Ninety six cats were randomly divided into 4 groups: group Ⅰ being placed under cardiopulmonary bypass (CPB) without aortic cross clamping (ACC), group Ⅱ receiving no cardioplegia during ACC, group Ⅲ and group Ⅳ receiving antegrade intermittent cold crystalloid cardioplegia and continuous antegrade warm blood cardioplegia during ACC, respectively. Cardiac performances and the activities of ATPase on myocardial cell membrane were detected. The alterations of myocardial cell membrane protein particles were observed by means of freeze replica techniques. Results No significant changes were found in group Ⅰ during CPB. There was a markedly decrease of NaK ATPase activities in groups Ⅱ, Ⅲ and Ⅳ during ACC ( P <0.01). The activities of CaMg ATPase and the numbers of myocardial membrane protein particles were decreased significantly in group Ⅱ and group Ⅲ ( P <0.05). After reperfusion although the CaMg ATPase activities and cardiac performances recovered to normal levels in group Ⅳ, the NaK ATPase activities and the numbers of myocardial membrane protein particles were still at lower levels. Conclusion CPB without ACC does not exert detrimental efforts on the cardiac performances and myocardial membrane proteins. Continuous antegrade warm blood cardioplegia administration offers satisfactory myocardial protective effects.
