The study of brain proton magnetic resonance spectroscopy(~1H-MRS) in patients with mild cognitive impairment

摘要 目的:探讨氢质子磁共振波谱(1H-MRS)对遗忘型轻度认知功能损害(aMCI)患者,Alzheimer病(AD)和正常对照组的诊断和鉴别诊断的作用。方法:采用德国Siemens公司生产的Novus 1.5T磁共振成像系统,对入选的研究对象行相应的常规MRI扫描,脑内感兴趣区的1H-MRS采集,采用SVS-SE-30序列,对12例aMCI患者、12例AD患者和12例性别、年龄、文化程度相匹配健康对照的双侧内侧颞叶和左后扣带回1H-MRS进行分析,比较各组间N-乙酰天门冬氨酸(NAA)/肌酸(Cr),胆碱(Cho)/Cr,肌醇(mI)/Cr,NAA/mI及不对称指数的差别。结果:左内侧颞叶aMCI和AD组的NAA/Cr均明显低于NC组(P<0.05),AD组与aMCI组比较无显著性改变(P>0.05);aMCI组和AD组的NAA/mI低于NC组(P<0.05),AD组与aMCI组比较无显著变化(P>0.05)。右内侧颞叶AD组和aMCI的NAA/mI显著低于NC组(P<0.05)。aMCI、AD和NC组左扣带回后部NAA/Cr、Cho/Cr、mI/Cr及NAA/mI差异均无显著变化。双内侧颞叶aMCI组与NC组代谢物不对称指数比较发现,aMCI组与NC组比较差异显著(P<0.05),余代谢物比值差异无显著变化(P>0.05)。结论:左内侧颞叶NAA/Cr的变化在aMCI、AD与NC鉴别诊断中有重要价值;双内侧颞叶NAA/mI比值的变化早于NAA/Cr的改变,可能神经胶质的增生早于神经元的丢失。1H-MRS是aMCI、AD与NC的诊断、鉴别诊断和监测的有用工具。 Objective:To evaluate the roles of proton magnetic resonance spectroscopy(1H-MRS) in the diagnosis and differential diagnosis of amnestic mild cognitive impairment(aMCI),Alzheimer disease(AD) and normal control group.Methods:Normal scanning,1H-MRS of VOI are acquired by Siemens Novus 1.5T MRI system.A 1H-MRS was performed in the bilateral medial temporal lobes(MITS) and left posterior cingulated contex(PCC) of the 12 aMCI patients,12 AD patients and 12 healthy age and sex-matched controls using single voxel sequence mode(SVS-SE-30).The ratios of NAA/Cr,Cho/Cr,mI/Cr,NAA/mI and metabolite asymmetries were compared among the groups.Results:NAA/Cr of left medial temporal lobe in the aMCI and AD groups were lower than that of the NC group with statistical significance(P0.05),AD group and the aMCI group had no statistically significant difference(P0.05);NAA/mI in aMCI and AD group was lower than that in the NC group with statistical significance(P0.05).There was no statistically significant difference in AD group and the aMCI group(P0.05).NAA/mI of right medial temporal lobe in AD and aMCI group was lower than that of the NC group with statistical significance(P0.05).There were no statistical significance of NAA/Cr,Cho/Cr,mI/Cr and NAA/mI located in left posterior cingulate gyrus of aMCI,AD and NC groups.Metabolites asymmetry index of two sides of medial temporal lobe of aMCI group and NC group had statistically significant difference(P0.05),metabolites asymmetry index of other position had no significant difference.Conclusion:The changes of NAA/Cr in the left medial temporal lobe play an important role in differential diagnosis of aMCI,AD and NC;the reduction of NAA/mI in bilateral medial temporal lobes is earlier than NAA/Cr in aMCI due to the reason that hyperplasia of neuroglial is earlier than the loss of neurons.1H-MRS is a useful method in monitoring,diagnosis and differential diagnosis of aMCI,AD and NC.
