摘要
目的:探讨室间隔缺损的诊断、手术方式及围手术期处理。方法:手术治疗室间隔缺损421例,其中补片修复290例,直接缝合修补131例。合并畸形135例,需行矫正者120例均给予手术矫正。结果:早期死亡4例,3例死于肺动脉高压危象,1例死于心律失常,术后4个月死亡1例,死于心力衰竭,死亡率1.18%。结论:室间隔缺损为先天性心血管畸形的常见病,手术效果好,但对于室缺合并其他畸形者强调术前明确诊断、掌握手术指证,术中详细探查,围手术期心肺支持与保护。
Objective:To investigate the diagnosis of ventricular septal defect,surgical and perioperative management.Methods:The surgical treatment of 421 cases of ventricular septal defect,290 cases in which surgical repair,direct suture repair of 131 cases.Malformation of 135 cases,which needed correction by surgical correction of 120 cases were given.Results:Early death in 4 cases,3 patients died of pulmonary hypertension crisis,1 patient died of arrhythmia,1 patient died after surgery in April,died of heart failure,mortality rate of 1.18%.Conclusion:The ventricular septal defect with congenital cardiovascular malformations common and effective surgery,but for ventricular septal defect combined with other malformations stressed the preoperative diagnosis,operation are mastered,more exploratory surgery and perioperative cardiopulmonary support and protection.
