摘要
目的探讨精索静脉曲张高位结扎术中,改进手术方法对提高疗效、减少术后并发症的作用。方法对670例患者选用改良Palomo术式,行双侧结扎,不保留动脉,对其疗效和并发症的影响,进行临床研究。结果本组670例术后精子密度的改善率在不同的分组中分别为94.6%、83.0%和34.9%;术后活动率的改善率分别为93.0%、85.1%和27.9%;术后睾丸鞘膜积液发生率0.6%,复发率0,睾丸坏死或萎缩发生率0。结论对精索静脉曲张引起男性不育患者采用改良Palomo式精索静脉结扎术,行双侧结扎,不保留动脉,可以提高疗效,降低术后并发症的发生率。
Objective To study on the role of improved surgical method in enhancing curative effect and reducing postoperative complication in palomo varicocele ligation. Methods 670 cases were selected to accept modified Palomo operation with bilateral ligation, without retaining the artery,and a clinical research is made on its curative effect and complications. Results The postoperative improvement rate of sperm density of the 670 cases in different groups were 94.6%, 83.0% and 34.9% respectively;the improvement rate of postoperative activity ratio were 93 % ,85.1% and 27.9% respectively;the postoperative incidence rate of hydrocele testis was 0.6%, the recurrence rate was 0, the incidence rate of necrosis or orchiatrophy of testis was 0. Conclusion The use of modified Palomo varicocele ligation with bilateral ligation without retaining the artery can enhance the curative effectiveness and reduce the incidence of postoperative complications in treatment of male infertility caused by varicocele.
