摘要
目的探讨女性尸体骶前纵韧带分段进行离体的生物力学试验,为阴道骶骨固定术手术固定位点提供力学依据。方法2008年9月至2009年4月在广州医学院选用成年女性尸体骶前纵韧带16份,分为S1段(16例)、S2段(16例)及S3-5段(16例),以数显卡尺测量试样的厚度,然后进行拉伸试验,获取女性尸体骶前纵韧带生物力学数据。结果骶前纵韧带S1段、S2段、S3~5段厚度分别为(0.59±0.12)mm、(0.37±0.08)mm和(0.22±0.04)mm,厚度渐小,P〈0.05,差异有统计学意义;而最大载荷分别为(38.59±4.65)N、(22.74±5.21)N和(11.30±2.99)N;应力分别为(30.27±2.71)MPa、(20.02±1.25)MPa和(16.69±2.34)MPa;弹性模量分另q为(60.32±7.56)MPa、(30.08±5.89)MPa和(20.11±6.72)MPa;3组间应变差异无统计学意义,P〉0.05;最大载荷、应力、弹性模量依次递减,P〈0.05,差异有统计学意义。结论骶前纵韧带在S1处为厚度最厚、强度及刚度最大部位,S1的骶前纵韧带为阴道骶骨固定术最佳的缝合固定部位。
Objective In vitro biomechanieal test was performing on difference segments of the adult female cadavers anterior longitudinal ligament of sacral vertebrae. The aim of our research was to supply biomechanieal data for the fixed position on the sacrum of saerocolpopexy. Methods Between September 2008 and April 2009, Guangzhou Medical College Setected 16 pieces of anterior longitudinal ligament of sacral vertebrae whieh were took from 16 adult female eadavers, were trimmed for the S1 segment (16 cases), S2 segment (16 cases) and S3-5 segment (16 eases) After measuring the thickness of the specimens with the digital calipers, the speeimens were placed into the testing machine to deal with the tensile test. Obtain biomechanieal data of female cadaver anterior longitudinal ligament of sacral vertebrae. Results The thickness of anterior longitudinal ligament was different in different segments. The thickness of S1 segment, S2 segment, S3 -5 segment respectively were: (0. 59 s0. 12) mm, (0. 37±0.08) mm and (0.22 ±0.04) ram. It was declining which was a significant difference ( P 〈 0. 05 ). The maximum load of them respectively were ( 38. 59 ± 4.65 ) N, ( 22. 74 ± 5.21 ) N and ( 11.30 ± 2.99) N ; the stress respectively were ( 30. 27 ± 2. 71 ) MPa, ( 20. 02 ± 1.25 ) MPa and ( 16.69 ±2. 34) MPa;and the modulus of elasticity respectively were (60. 32 ± 7.56) MPa, (30. 08 ±5.89) MPa and (20. 11 ± 6. 72 ) MPa. 3 groups had no significant difference in the strain difference ( P 〉 0. 05 ). The maximum load, the stress and the modulus of elasticity were decreasing( P 〈 0. 05) ,there was a significant difference. Conclusion The anterior longitudinal ligament of sacral vertebrae of S1 segment is the thickest, with the greatest strength and stiffness. The anterior longitudinal ligament of sacral vertebrae of S1 segment is the best reasonable fixation position for saeroeolpopexy, which can provide stronger fixation strength and stiffness.
