摘要
目的探讨成人Still(adult-onsetstill′s diseases,AOSD)病的临床特点。方法采用1992年日本成人Still病研究委员会提出的诊断标准。回顾性分析63例成人Still病患者的临床资料和诊治过程。结果临床表现为98.4%有发热、90.5%有皮疹,79.4%有关节痛,55.6%有咽喉痛,实验室检查:93.7%伴有白细胞升高。63例患者血清铁蛋白均升高,血培养均为阴性。行骨髓检查34例,示感染性骨髓象者18例。治疗上首选非甾体类抗炎药和糖皮质类固醇激素,63例患者均获缓解。结论成人Still病临床特征以发热、皮疹、关节症状和咽痛为特征。实验室检查可见白细胞、血清铁蛋白升高、血培养阴性,诊断必须排除其他疾病,是一种独立的疾病,大多数患者对皮质激素反应良好。
Objective To investigate the clinical characteristics of adult Still＇s disease （AOSD） . Methods According to the diagnostic criteria of 1992 Adult Still＇s Disease Research Committee of Japan, the clinical data of 63 patients with AOSD were retrospectively analyzed. Results Clinically 98.4% of the cases had fever, 90. 5% had rash, 79.4% had arthralgia and 55.6% had sore throat, and 93.7% had an elevation of leukocyte. All the 63 patients had an elevation of serum ferritin and negative blood bacterial cultures. Among 34 patients who had bone marrow aspiration, 18 showed infectivity. Glucocortisteroid and non - steroidal anti - inflammatory drug （ NSAID ） were the first choice and 63 patients were released after treatment. Conclnsion The clinical features of AOSD are fever, rash, arthralgia and sore throat. Laboratory test showed elevations of leukocyte and serum ferritin. Blood culture was all negative. The diagnosis must be ruled out other diseases and it is an independent disease. Most patients respond to corticosteroids well.
