摘要
目的建立一种简单、稳定的大鼠工作型腹部异位心脏移植模型。方法将供体肺动脉与供体左心房吻合,供心升主动脉与受体腹主动脉行端侧吻合,受体血液经供心冠状动脉进入,在供心内循环后,由左心室排出,经供心主动脉吻合口进入受体腹主动脉。结果 20例实验成功17例。成功率85%,供心平均缺血时间为(33.2±3.7)min。失败的原因有吻合口出血,心肌保护不佳。术后1个月心脏彩超检查见供心二尖瓣口和主动脉瓣口均有血流通过。结论大鼠工作型腹部心脏移植模型更接近心脏生理,可更好地应用于实验研究;且操作简单实用,甚至可不需手术显微镜,易于推广。
Objective To establish an abdominal heterotopic working-heart transplantation model in rats. Methods Donor's heart was transplanted by using end-to-side anastomosis between the donor's pulmonary artery and the donor's left atrium,and between the donor's ascending aorta and the recipient's abdominal aorta pulmonary artery.The recipient's blood entered into me donor's hearts by the donor's coronary artery, circulated in the heart,flowed out by the donor's left ventricular ejection,and returned the recipient's abdominal aorta through the anastomosis of the donor's ascending aorta to the recipient's abdominal aorta. Results The mean ischemic time was(33.2±3.7) minutes in 17 successful cases in all 20 experiments. The successful rate was 85%.Failure reasons included anastomotic bleeding,poor myocardial protection, and so on.Echocardiography examination revealed there was blood flow through the donor's mitral orifice and aortic valve orifice a month postoperatively.Conclusion Rat abdominal heterotopic working-heart transplantation models are closer to the normal heart physiologically and can be better used in experimental studies.This operation is simple and practical,even without using surgical microscope,and easy to spread.
