扩散加权成像对前列腺癌骨转移的诊断价值

摘要 目的:评价磁共振扩散加权成像(DWI)对前列腺癌骨转移的诊断价值。方法:分析30例前列腺癌(PCa)骨转移患者DWI和ADC图表现,分别测量前列腺癌、骨转移灶和正常骨的ADC值,并进行统计学分析,所有患者均行同位素全身骨显像检查。结果:30例PCa骨转移患者MRI共检出骨盆转移灶57个,PCa癌灶和骨转移灶在DWI上呈高信号影。前列腺癌、骨转移灶和正常骨的ADC值分别为(0.930±0.091)×10-3 mm2/s、(0.926±0.110)×10-3 mm2/s和(0.218±0.098)×10-3 mm2/s。PCa骨转移灶的ADC值明显高于正常骨(t=3.081,P<0.05),但与PCa癌灶的ADC值之间的差异无统计学意义(t=0.909,P>0.05)。结论:DWI有助于前列腺癌骨转移的诊断。 Objective To evaluate the diagnostic value of diffusion weighted imaging(DWI) and the apparent diffusion coefficient(ADC) in demonstrating PCa bone metastasis.Methods 30 patients with PCa bone metastasis were scanned with DWI respectively.DWI appearance of all PCa foci,bone metastasis and normal bone were analyzed,and their ADC values were measured.All patients underwent bone scan.Results The PCa foci and bone metastasis showed high signal intensity on DWI.The mean ADCs of PCa,bone metastasis and normal bone were(0.930±0.091)×10-3mm2/s,(0.926±0.110) ×10-3mm2/s and(0.218±0.098) ×10-3mm2/s respectively.The ADC values of bone metastasis were higher than those of normal bone(t=3.081,P〈0.05).There was no significant difference between the ADC values of PCa and bone metastasis(t=0.909,P〉0.05).Conclusion DWI plays a useful role in diagnosis of PCa bone metastasis.
