拇趾趾背神经筋膜蒂皮瓣修复拇趾趾端缺损 被引量:5

摘要 目的:报道利用拇趾趾背神经筋膜蒂皮瓣修复拇趾趾端缺损的方法和临床疗效。方法:自2007年5月至2009年9月,对7例拇趾趾端缺损患者,采用拇趾趾背神经筋膜蒂皮瓣修复。拇趾趾端缺损面积2.0cm×1.8cm~3.0cm×2.5cm,切取皮瓣的面积为2.2cm×2.0cm~3.5cm×2.8cm,趾背供区采用下腹部全厚皮片植皮。结果:本组7例,皮瓣全部存活,术后随访6~24月,平均14个月,拇趾趾端的外形基本恢复,有效地保留了拇趾的长度。趾甲生长良好、无钩甲嵌甲等畸形,足部行走功能有无明显影响,趾背供区植皮存活无破溃及瘢痕挛缩。结论:拇趾趾背神经筋膜蒂皮瓣具有切取安全,操作简单、血供可靠等优点,是修复拇趾趾端缺损的较好方法。 Objective To report the methods and clinical effects of utilization of dorsal digital nerve fascial pedicled flap of halluce repairing defects of halluce toe end.Methods From May 2007 to September 2009,7 patients with halluce toe end defects were repaired by dorsal digital nerve fascia peidicled flap.The area of halluce toe end was 2.0cm × 1.8cm~3.0cm × 2.5cm,the area of cutting flap was 2.2cm× 2.0cm~3.5cm×2.8cm,the area of dorsal digital defects were adopted from the lower abdomen for full-thickness skin grafting.Results There were 7 cases in the group,and all flaps were survived,also followed up for 6~24 months after operation.The average time was 14 months when the shape of halluce toe end could basically recovey.Also the operation was effectively keep the length of the halluce,and nails grew well without deformity as hooked nails or embedded nails.Moreover there was no significant influences on foot walking function,and dorsal digital skin grafting was survived without rupture and scar contracture.Conclusion Dorsal digital nerve fascial pedicled flaps was safe and simple for cutting and practice.And it had the advantage of reliable blood supply.It was a better method for repairing halluce toe end defects.
