摘要
目的了解吉林省延边州朝鲜族居民原发性高血压(EH)的遗传特征及其相关危险因素,为采取相应地干预措施提供参考依据。方法采用以医院为基础病例对照研究方法对2006年6月-2008年12月在吉林省延边州各大医院就诊的270例EH患者及同期体检的420名非高血压健康人群进行EH相关因素的问卷调查及生理和生化指标检测。结果延边州朝鲜族EH患者遗传度为57.0%,其中男性为62.3%,女性为54.6%;病例组家系一级亲属、家系男性一级亲属和家系女性一级亲属的患病率分别为30.45%、35.27%和27.13%,均高于对照组的15.13%、17.27%和13.11%,差异均有统计学意义(P<0.001);延边州朝鲜族EH患者分离比为0.16,符合多基因遗传病特征(分离比<0.25);多因素条件Logistic回归分析结果表明,有高血压家族史、肥胖、饮酒、高盐饮食、血浆脂联素≤4.68μg/mL是延边州朝鲜族居民患EH的主要危险因素。结论延边州朝鲜族居民EH患者有明显遗传倾向,具有多基因遗传特征;有高血压家族史、肥胖、饮酒过量、高盐饮食、血浆低脂联素的朝鲜族居民患EH的危险性较大。
Objective To explore the genetic characteristics and risk factors of essential hypertension among Chinese Korean in Yanbian area.Methods Totally 270 essential hypertension（EH） patients of Yanbian Chinese Korean were selected as case-group and 420 health individuals as control-group.The information on general condition,living style,disease history,and familial history were collected with a fcae-to-face interview among the subjects.The height,weight,waist circumference,hip circumference,blood pressure,body fat percentage,total cholesterol（TC）,triglyceride（TG）,low-density lipoprotein cholesterol（LDL-C）,high-density lipoprotein cholesterol（HDL-C）,fasting plasma glucose（FPG）,plasma insulin,and adiponectin of the subjects were measured.All data were analyzed with SPSS17.0 software.Multiple factor logistic regression and χ2 test were used for analysis.Results The prevalence rate of EH among first-degree relatives（30.44%） in EH was higher than that of in the control group（15.13%,P0.05）.The heredity of EH among Chinese Korean in Yanbian was 57%.The seperation ratio of EH（0.16） showed a polygenic inheritance in heredity mode.EH family history,body mass index（BMI）,body fat rate,alcohol drinking,salt intake,and the level of plasma adiponectin were associated with EH by multiple factor logistic regression analysis.Conclusion The prevalence of EH shows an obviousl hereditary tendency and the character of polygentic disease among Yanbian Chinses Korean.EH family history,obesity,alochol drinking,higher intake of salt,and lower plasma adiponectin are important risk factors of EH for Chinese Korean in Yanbian area.
