摘要
目的分析孔源性视网膜脱离（ＲＲＤ）不放液手术的效果和影响视网膜下液（ＳＲＦ）吸收、视网膜复位的因素。方法１２６例（１３０眼）ＲＲＤ采用视网膜冷凝、巩膜环扎、外加压、不放液的手术方法。结果１次手术后１１９眼视网膜完全复位（９１．５％），２眼再次不放液手术后视网膜复位，总复位率为９３．１％；ＳＲＦ术后２４ｈ吸收３７眼（２８．４％），７天内吸收１０１眼（７７．７％）；术后视力明显提高。结论不放液手术效果与放液手术相同、且避免了放液并发症；影响手术效果的主要因素是手术前ＰＶＲ程度和手术后裂孔是否封闭。
Objective To analyze the effect of surgery without drainage of subretinal fluid(SRF)and the factors influencing the absorption of SRF or reattachment of retina in rhegmatogenous retinal detachment(RRD). Methods 126 cases(130 eyes)with RRD were treated by cryoretinopexy,scleral encircling,external pressure and surgery without drainage of SRF. Results The retina was completely reattached in 119 eyes(91.5%)after primary surgery.Total reattachment rate was 93.1% after further surgery without drainage of SRF in 2 eyes.SRF was absorbed within 24h in 37 eyes(28.4%)and within 7d in 101 eyes(77.7%)after surgery.The postoperative visual acuity showed a marked improvement in most patients. Conclusion The effect of surgery without drainage of SRF is similar to that of surgery with drainage of SRF,thus the complications of draining SRF can be avoided.The essential factors influencing the effect are preoperative extent of PVR and postoperative closing of holes.
