摘要
目的:探讨传染病医院口腔科常见急诊种类和特点。方法:回顾性分析2008年1月-2009年12月口腔科急会诊的544例住院患者临床资料。其中牙龈出血204例;口腔溃疡187例;牙髓炎69例;根尖炎43例;智齿冠周炎41例,在进行一般治疗的基础上,给予口腔科专业处理。结果:牙龈出血204例经用无菌棉球压迫30min,166例出血停止。38例用无菌棉球压迫后仍出血不止,改用明胶海绵揉成小米粒状填塞牙龈沟内外加无菌棉球压迫,补充维生素K、血小板、止血药物等纠正凝血功能,12h内出血全部停止。口腔溃疡141例在7d内愈合,34例合并口腔霉菌感染,经加用抗真菌治疗后逐渐愈合,12例口腔溃疡持续30d未愈,均为艾滋病患者(CD4T淋巴细胞<50cell/μl)。智齿冠周炎经治疗后3～7d临床症状基本消失。急性牙髓炎和急性根尖炎立即作开髓治疗83例症状缓解,随后行根管治疗痊愈;29例加静脉应用抗生素治疗后痊愈。结论:肝炎、HIV/AIDS患者合并口腔急症主要病种是出血和炎症。处理这些口腔科急症,不仅需要口腔专科技术而且需要传染病学相关知识。
Objective：To discuss the common types and features of emergency in the stomatology department of infectious hospital.Method：The data of 544cases of patients with dental emergency in our hospital from Jan 2008to Dec 2009were analysis retrospectively.There were 204cases of gingival bleeding,187cases of oral ulcer,69cases of pulpitis,43cases of apicitis,41cases of ericoronitis of the wisdom tooth in our study.All patients were given basic treatment and professional dental treatment.Result：166in 204cases of gingival bleeding were stopped by cotton ball press within 30minutes.The bleeding were not stopped in 38cases until by using small gelatin sponge obturated in the gingival sulcus,plus intravenous injection vitamin K,platelet, and hemostatic to adjust blood coagulation function.All the bleeding was stopped in 12hours.141patients with oral ulcer heal in 7days after treatment.There were 34cases complicated with oral fungal infection healed by antifungal therapy.12cases of AIDS patients with oral ulcer were not healed in 30days.The symptoms of Pericoronitis of the third molar of the mandible were disappeared in 3to 7days after treatment.The symptoms of 83cases with acute pulpitis and acute apicitis immediately relieved by open tooth pulp treatment.They were fully recovered after root canal therapy.The other 29patients were recovered by adding intravenous antibiotics treatment.Conclusion：The main type of stomatological emergency in infectious hospital was bleeding and inflammation.To treat stomatological emergency in infectious hospital not only need special technical of stomatol-ogy but also require knowledge of infectious diseases.
