摘要 目的探讨腹腔镜辅助阴式全子宫切除术(LAVH)加阴道顶端悬吊重建盆底预防子宫切除术后盆底器官脱垂的临床效果。方法 2008年6月~2010年6月,收集因子宫良性疾病行LAVH,并联合盆底重建悬吊阴道顶的病例22例,并抽取30例未行盆底悬吊的LAVH患者作为对照组,分析术中术后的并发症,并随访手术后阴道穹隆脱垂和性生活等情况。结果所有手术均顺利进行,无术中并发症,且在随访时间内临床症状均减轻或消失。研究组术后无阴道穹隆脱垂,术后6个月性生活质量评分较对照组高,差异有统计学意义(P<0.05)。手术时间、出血量、住院时间两组比较,差异无统计学意义(P>0.05)。对照组术后有2例出现阴道顶脱垂。结论 LAVH加阴道顶端悬吊重建盆底预防子宫切除术后盆底器官脱垂安全可靠,患者耐受性好,近期疗效稳定。 Objective laparoscopic assisted vaginal hysterectomy(LAVH) plus vaginal reconstruction of the pelvic floor to prevent the top suspension after hysterectomy pelvic organ prolapse results.Methods June 2008 ~ June 2010,our department because of gynecological benign diseases operating LAVH 22 cases combined suspension the top of vaginal,and collected 30 cases in the LAVH of not pelvic suspension as a control group of patients,analysis intraoperative and postoperative complications,and follow-up after surgery vaginal vault prolapse and sexual life and so on.Results All operations were conducted smoothly without intraoperative complications and follow-up time in the clinical symptoms were relieved or disappeared.Study group were free of vaginal vault prolapse,after 6 months of quality of life scores than the control group,the difference was statistically significant(P 0.05).Operation time,blood loss,hospital stay between the two groups,the difference was not statistically significant(p 0.05).The control group were 2 patients had prolapse of the vaginal top.Conclusion LAVH plus vaginal reconstruction of the pelvic floor to prevent the top suspension pelvic organ prolapse after hysterectomy safe and well tolerated in patients with recent efficacy and stability.
