摘要
目的:探讨动态增强MRI对颅内原发性肿瘤的诊断价值。材料与方法:使用1.0TMRI系统(MagnetomImpact,Siemens)SE序列对72例颅内原发性肿瘤进行动态增强扫描,分析各种肿瘤的对比增强率时间(CERT)曲线类型、增强峰值(EP)及最大对比增强率(MCER)。结果:EP在90秒内见于3例血管瘤型脑膜瘤、1例血管母细胞瘤及1例脉络膜乳头状瘤;26例星形细胞瘤仅4例Ⅲ、Ⅳ级的EP在180秒内,而25例脑膜瘤中的13例EP在180秒内,P值<0.01;血管瘤型脑膜瘤、神经鞘瘤、血管母细胞瘤及脉络膜乳头状瘤的EP及MCER明显高于其他肿瘤。结论:动态增强MRI的CERT曲线类型、EP及MCER有助于部分颅内原发性肿瘤的组织学诊断和富血管度评价。
Objective: To evaluate the dynamic enhanced MRI in diagnosing of primary intracranial tumors.Materials and Methods:Dynamic spin echo MRI was performed in 72 patients with primary intracranial tumors using a 1.0T MR unit (Magnetom Impact, Siemens). Contrast enhancement ratio time (CER T) curves, enhancement peak (EP) and maximum contrast enhancement ratio (MCER) of the tumor were analyzed.Results:EP appeared within 90 seconds was seen in angioblastic meningioma (n=3), hemangioblastoma (n=1) and choroid plexus papiloma (n=1). Thirteen out of 25 cases with meningioma showed EP within 180 seconds, while only 4 (grade 3 or 4) out of 26 cases with astrocytoma did within the same time (P<0.01). EP and MCER of the angioblastic meningioma, neurilemmoma, hemangioblastoma and choroid plexus papiloma was much higher than that of the other tumors.Conclusion: The CER T curves, EP and MCER with dynamic enhanced MRI are very helpful for the histologic diagnosis of some primary intracranial tumors and for the assessment of the vascularity of the tumor.
