摘要
目的报告应用肌腱移植加生物膜包裹修复Ⅱ区屈肌腱陈旧性断裂的临床效果。方法对27例36指屈肌腱陈旧性断裂患者,采用肌腱移植加医用可吸收生物膜包裹的方法进行修复,术后进行系统的康复锻炼,观察临床效果。结果27例36指伤口均工期愈合,术后随访4~36个月(平均13个月),按Strickland和Glogovac评价标准进行评价:优10指,良17指,一般和差共9指;优良率为75%。术后元一例发生肌腱再次断裂。9指效果一般和差者再次行肌腱松解术后3个月随访,优2指、良4指、一般和差共3指,总的优良率为92%。结论肌腱移植加可吸收生物膜包裹修复Ⅱ区屈肌腱陈旧性断裂,可获得较满意的效果。肌腱牢固的缝合、可吸收生物膜的应用及术后早期系统的康复锻炼是获得良好效果的关键因素。
Objective To report the clinical results of repairing flexor tendon rupture in zone Ⅱ with seprafllm and tendon graft. Methods Twenty-seven patients with chronic flexor tendon rupture in 36 fingers were treated with tendon graft and sepralllm, the medical absorbable biomembmne. Postoperatively systematic rehabilitation exercises were carried out. Clinical results were observed. Results Primary healing was achieved in all of the 36 fingers of the 27 patients. Postoperative follow-up ranged from 4 to 36 months, with an average of 13 months. Evaluation using Striekland and Glogovac criteria revealed excellent results in I0 fingers, good in 17 fingers, and fair and poor in 9 fingers. The satisfactory rate was 75 %. Recurrence of tendon rupture was absent. Tenolysis was carried out in the 9 fingers with poor results 3 months later. Another 3-month follow-up study that reevaluated the functions of the fingers showed excellent results in 2 fingers, good in 4 fingers, and fair and poor in 3 fingers. The satisfactory rate was 92%. Conclusion Treatment of chronic flexor tendon rupture in zone Ⅱ with seprafilm and tendon graft led to satisfying results. Secure suturing of the tendon, application of absorbable biomembranes and systematic postoperative rehabilitation exercises are the key to good clinical effect.
