伴新月体形成的IgA肾病患者尿单核细胞趋化因子1的检测及意义 被引量:1

摘要 目的探讨IgA肾病(IgAN)患者尿单核细胞趋化因子1(MCP-1)水平及其与IgAN肾小球新月体病变和临床各指标的关系。方法采用酶联免疫吸附测定(ELISA)法检测正常人及IgAN患者尿液中MCP-1的水平,并进行组间比较。并分别与病理中新月体指数及临床指标进行相关性分析。结果 IgAN患者尿中MCP-1水平较正常对照组显著升高,中位数和四分位数间距为29.30(73.49)ng/L vs 7.16(7.82)ng/L(P<0.01)。新月体指数≥50%组的MCP-1水平与无新月体组、新月体指数≤25%组比较均显著升高44.92(105.43)ng/L vs 25.39(42.49)ng/L,28.32(65.42)ng/L(P<0.05)。随着新月体指数的增加尿MCP-1水平逐渐升高。细胞新月体指数≥25%组尿MCP-1水平较无新月体组显著升高(P<0.05)。尿MCP-1的水平与24小时尿蛋白定量正相关,与血清白蛋白负相关,与新月体指数及细胞新月体指数均呈正相关。新月体指数与尿蛋白定量呈正相关(P<0.01),与血清白蛋白负相关(P<0.05)。结论 IgAN患者尿液MCP-1水平显著升高,与肾病理新月体形成密切相关。 Objective To detect the urinary level of monocyte chemoattractant ptotein-1(MCP-1) in patients with IgA nephropathy(IgAN) and to analyze the relationship among MCP-1 in urine and the clinical and pathological features of patients,especially in the formation of crescent.Methods Enzyme-linked immunosorbent assay(ELISA) was applied to detect the levels of urinary MCP-1 of IgAN patients and normal subjects(control group).Also comparisons were made between IgAN groups and the control group.Clinical parameters were detected at the same time.Correlation analysis was processed between ELISA result,crescent index and clinical parameters respectively.Results The urinary level of MCP-1 in IgAN group increased significantly compared with that of control group,M(QR) value 29.30(73.49) ng/L vs 7.16(7.82) ng/L(P0.01).The level of urinary MCP-1 in index of crescent ≥50% group increased compared with that of non crescent group and the index of crescent ≤25% group 44.92(105.43) ng/L vs 25.39(42.49) ng/L,28.32(65.42) ng/L(P0.05).The level of urinary MCP-1 of index of cellular crescent ≥25% group increased compared with that of non crescent group(P0.05).The level of urinary MCP-1 was positively correlated with 24 h urine protein quantitation(P0.05) and negatively correlated with serum albumin(P0.01).Urinary levels of MCP-1 were positively correlated with the index of crescent(P0.01) and the index of cellular crescent(P0.05).The index of crescent was positively correlated with 24 h urine protein quantitation(P0.01) and negatively correlated with serum albumin(P0.05).Conclusion The levels of urinary MCP-1 increased significantly in IgAN patients.The level of MCP-1 was positively correlated with the formation of ccrescent.
