摘要
目的评价经鼻高频喷射通气(HFJV)治疗急性心源性肺水肿的疗效。方法急性心源性肺水肿患者98例被随机分为HFJV组和对照组,HFJV组(48例)在传统常规治疗同时加用经鼻高频喷射通气治疗,对照组(50例)在传统常规治疗上加用鼻导管给氧治疗。结果 HFJV组通气治疗后,44例患者临床症状明显改善,呼吸频率(RR)、心率(HR)、血压(BP)等参数与治疗前比较有显著好转(P<0.01),血气分析中pH、动脉血氧分压(PaO_2)、动脉血二氧化碳分压(PaCO_2)、动脉血氧饱和度(SaO_2)等参数与治疗前比较均有显著好转(P<0.01);HR、RR、PaO_2、SaO_2较对照组显著好转(P<0.05),总有效率为91.7%,明显优于常规治疗组(76.0%)。结论经鼻HFJV治疗急性左心功能衰竭能迅速纠正缺氧、改善心功能,可作为抢救的一种安全、有效方法。
Objective To asses the efficacy of high frequency jet ventilation through the nose in the treatment of acute cardiogenic pulmonary edema. Methods 98 patients with acute cardiogenic pulmonary edema were divided into two groups randomly: HFJV group and control group. The 48 patients in the HFJV group were treated with high frequency jet ventilation through the nose in addition to conventional treatment. Another 50 patients in the control group were treated by nasal pipe with oxygen in addition to conventional treatment. Results After HFJV, the clinical symptoms of 44 patients in the HFJV group had been improved significantly, their heart rate(HR), respiration rate(RR), blood pressure showed a significant improvement( P 〈 0. 01 ) compared with those before HFJV, and their arterial blood gas parameters(pH, PaO2, PaCO2, SaO2) showed a significant improvement( P 〈0.01) too, and RR, HR, PaO2, SaO2 were improved compared with control group( P 〈0.05). The total efficiency rate of HFJV group was 91.7%, more than that of control group(76.0% ). Conclusion High frequency jet ventilation through the nose could improve hypoxemia and cardiac function in patients with acute cardiogenic pulmonary edema, and it is safe and effective.
