摘要
目的 :研究脑中风急性期甘露醇用量和血糖浓度对血浆晶体渗透浓度的影响。方法 :测定 34例脑卒中急性期病人血浆电解质和葡萄糖的渗透浓度 ,计算同期甘露醇用量总数。设立HBSS、NBSS实验组和对照组。结果 :仅电解质形成的渗透浓度 ,三组间无明显差异 ,由葡萄糖形成的渗透浓度及总血浆晶体渗透浓度 ,HBSS明显高于NBSS和对照组。 2 0 %甘露醇用量与HBSS的渗透浓度呈高度的直线正相关关系 (r=0 72 7,P <0 0 0 1)。结论 :对脑卒中急性期运用 2 0 %甘露醇降颅压时 ,除适当补充水和电解质外 ,需积极有效地控制高血糖 ,不盲目大量使用甘露醇。
Aim:To study the effects of total amount of mannitol given and concentration of blood sugar on plasma crystalloid osmolality (PCO) of patients with acute stroke.Methods:To measure the plasma osmolality of electrolytes and that of blood sugar in 34 patients with acute stroke.The total amount of mannitol given was calculated simultaneously.Patients were divided into three groups:HBSS group,NBSS group and control group.Results:There is no significant difference in PCO of electrolytes among three groups.The mean PCO of blood sugar and the mean total PCO in HBSS group were higher than that in NBSS group as well as control group.The PCO was positively correlated with the amount of 20% solution of mannitol given in HBSS group(r=0 727,P<0 001).Conclusion:Apart from complementing H 2O and electrolytes,we must effectively control the level of blood sugar and do not abuse the mannitol when we use 20% solution of mannitol to reduce intracranical pressure of the patients with stroke at acute stage.
