摘要
目的探讨粘连性小瞳孔下白内障的手术方法。方法通过26例（27眼）粘连性小瞳孔下白内障术中应用粘弹剂及人工晶状体调位钩进行钝性分离扩大瞳孔以协助完成超声乳化手术。结果全部病例均顺利完成撕囊及超声乳化术。后囊破裂1例（3．70％），角膜水肿12眼（44．44％），均于7天内消失。结论应用粘弹剂及人工晶状体调位钩扩大瞳孔是简单有效的方法，不损伤瞳孔括约肌，能保持圆形瞳孔，减少术中、术后并发症。
Objective To observe the surgical effect of cataract with small pupil. Methods On 27 eyes of 26 cases of cataract with small pupil, opend up small pupil with viscoelatstic and flexible iris retractor to perform phacoemulsification. Results All cases were done successfully continuous circular capsulorhexis and phacoemulsification. Posterior capsule ruptured in 1 eye （ 3.7% ）, cornea ; edema appeared in 12 eyes （44.44%） and it disappeard in 3 to 6 days. Conclusion Using viscoelatstic and flexible iris retractor for small pupil enlargement was a simple and efficient technique without sphincter injury. It can keep the circle pupil with few complications.
