摘要
目的评价18F-FDG PET显像在结直肠癌诊断、可疑复发或转移中的临床应用。方法对109例患者作18F-FDG PET检查,其中73例疑为结直肠癌,36例疑为结直肠癌术后复发或转移,并与同期CT对比。结果73例疑似为结直肠癌的患者确诊为结直肠癌72例,PET的诊断准确率98.6%。18F-FDG PET对肝转移诊断准确度95.9%,灵敏度84.6%,特异度98.3%。PET对腹腔淋巴结转移灵敏度49.1%,特异度73.3%,准确度43.7%。对36例结肠癌术后疑为复发或转移的患者,18F-FDG PET在检测肿瘤转移或复发的灵敏度92.3%,特异度80%,准确度88.9%,同期CT在检测肿瘤转移或复发的灵敏度85.7%,特异度40.7%,准确度66.7%。结论18F-FDG PET检查对可疑的结直肠癌有较高的临床诊断价值,肝转移和腹腔淋巴结转移灵敏度高,有利于临床制定治疗方案。18F-FDG PET显像较CT对可疑的结直肠癌术后复发和转移更敏感,有利于术后肿瘤复发或转移的判断,并为治疗提供依据。
【Objective】To evaluate the clinical application of 18F-FDG PET in colorectal cancer.【Methods】A total of 109 patients underwent 18F FDG PET imaging, of whom 73 patients were suspected to have colorectal cancer, 36 patients were suspected to have cancer recurrence and metastasis after radical operation. The results were contrasted with CT scan results at the same period. 【Results】In 73 suspected cases, 72 were confirmed as colorectal cancer. The overall accuracy was 98.6 %. 18F-FDG PET identified liver metastasis with a sensitivity of 84.6 % and specificity of 98.3 % and the overall accuracy of 95.9 %. The sensitivity for abdominal cavity lymph node metastasis was 49.1 %, specificity was 73.3 %, and accuracy was 43.7 %. 18F-FDG PET detected carcinoma metastasis and recurrence in 36 patients who were suspected to have metastasis and recurrence after radical operation of colorectal cancer with a sensitivity of 92.3 %, a specificity of 80 %, and the overall accuracy of 88.9 %. At the same time, the sensitivity of CT scan detection of metastasis and recurrence was 85.7%, the specificity was 40.7 %, the accuracy was 66.7 %.【Conclusion】18F-FDG PET has an important role in the diagnosis of colorectal cancer. The sensitivity for liver metastasis and abdominal cavity lymph node metastasis is higher. Compared with CT scan, 18F-FDG PET has a higher sensitivity in patients who are suspected of recurrence and metastasis after radical operation of colorectal cancers, so 18F-FDG PET is helpful for diagnosis and treatment of recurrence and metastasis of colorectal carcinoma
