间断血液透析滤过治疗尿毒症皮肤瘙痒的疗效观察 被引量:2

Efficacy of hemodialysis combined with hemodiafiltration on pruritus in mainteneance hemodialysis patients

摘要 目的探讨间断血液透析滤过治疗尿毒症患者皮肤瘙痒的临床疗效。方法尿毒症维持性血液透析(MHD)患者36例,随机均分两组:血液透析滤过(HDF)组每周血液透析2次,血液透析滤过1次;单纯血液透析(HD)组每周血液透析3次,均连续治疗12周。治疗前后测定甲状旁腺激素(PTH)及β2微球蛋白(β2-MG)水平,DirkRKuypers方法评定皮肤瘙痒症状。结果治疗12周后,HD组PTH、β2-MG水平和DirkRKuypers评分无明显变化(P>0.05),HDF组3项指标均明显降低(P<0.05),并低于HD组(P<0.01)。结论 MHD患者间断行HDF可增加PTH、β2-MG等中分子毒素的清除,明显减轻尿毒症患者顽固性皮肤瘙痒症状。 Objective To observe the effects of hemodialysis combined with hemodiafiltration on pruritus in maintenance hemodialysis patients.Methods Hemodialysis patients were divided randomly into two groups of HDF and HD with 18 cases each.Patients in group HDF were treated with hemodialysis twice and hemodiafiltration once per week and those in group HD were treated with hemodialysis three times per week.Hemodialysis lasted for 12 weeks.Before and after treatment,serum parathyroid hormone(PTH) and β2-microglobulin were measured and the change of pruritus was evaluated by Dirk R Kuypers scoring.Results Compared with group HD,serum PTH and β2-MG and Dirk R Kuypers score in group HDF were reduced and lower remarkably after 12 weeks(P〈0.01 and P〈0.05).Conclusion The treatment of hemodialysis combined with hemodiafiltration can effectively relieve skin pruritus in maintenance hemodialysis patients.
