摘要
目的总结双向格林手术治疗复杂先天性心脏病的临床经验。方法 2004年1月至2009年6月,河南省胸科医院采用双向格林手术治疗复杂先天性心脏病37例,年龄5个月～13岁,体质量5～27kg。20例手术在体外循环下进行,17例在非体外循环下进行。10例保留原肺动脉前向血流;19例附加Banding术,控制原肺动脉前向血流;8例切断原肺动脉前向血流。结果全组无手术死亡。术后发绀明显缓解,活动耐力增加。5例术后2～3年成功行Fontan术。结论双向格林手术是治疗复杂发绀型先天性心脏病的一种安全有效的术式。
Objective To summarize the clinic experience of bidirectional Glenn operation in treatment of complex congenital heart disease. Methods Bidirectional Glenn operation was performed on 37 cases with complex congenital heart disease in Henan Chest Hospital from January 2004 to June 2009. Age ranged from 5 months to 13 years. Weight ranged from 5 to 27kg. The operation was performed with cardiopulmonary bypass （CPB） in 20 cases, and without CPB in 17 cases. Native pulmonary antegrade blood flow was reserved in 10 cases, was controlled by banding procedure in 19 cases, and was cut off in 8 cases. Results There was no operative death. Cyanosis was alleviated obviously, and activity potential was improved in this group. 5 cases were performed Fontan procedure successfully 2～3 years later. Conclusion Bidirectional Glenn operation is a safe an effective method for complex cyanosis congenital disease. However the concrete procedure should be different by different patients.
