摘要
目的探讨抗核抗体荧光核型与特异性抗体之间的相关性,以期为临床提供更优、更经济的抗核抗体检测组合方式。方法应用欧蒙间接免疫荧光法(IIFA)检测抗核抗体(ANA)判断荧光核型,欧蒙印迹法(LIA)抗核抗体谱(ANAs)15项检测特异性抗体。对3783例患者IIFA-ANA、LIA-ANAs结果进行统计分析,根据检测结果分为IIFA-ANA+/LIA-ANAs-组1548例、IIFA-ANA+/LIA-ANAs+组2220例、IIFA-ANA-/LIA-ANAs+组15例。结果 3768例IIFA-ANA+患者,LIA-ANAs的阳性率为58.92%;1704例IIFA-ANA+/LIA-ANAs+单一荧光核型患者,IIFA-ANA与LIA-ANAs的符合率分别为核颗粒型82.61%、核均质82.80%、核仁型82.86%、着丝点型100%、胞浆型69.29%。IIFA-ANA-/LIA-ANAs+组降低IIFA-ANA稀释倍数后,出现与特异性抗体相对应的荧光核型。结论不能简单依靠IIFA-ANA的荧光核型推断特异性抗体;临床医生可以根据患者病情、IIFA-ANA荧光核型决定患者是否需要检测LIA-ANAs。
This study is aimed to investigate the correlation between antinuc lear antibody(ANA) fluorescence patterns and specific antibodies,so as to provide better and more economical combinations of ANA detection methods for clinical practice.ANA fluorescence patterns were detected by indirect immuno fluore scence assay(IIFA),while the specific antibodies were detected by Euroline assay(LIA) of 15 markers of ANAs.According to the results of IIFA-ANA and LIA-ANAs,3 783 patients were divided into IIFAANA+/LIA-ANAs-group(1 548 cases),IIFA-ANA+/LIA-ANAs+ group(2 220 cases),and IIFA-ANA-/LIA-ANAs+ group(15 cases).We found that the LIA-ANAs positive ratio of IIFA-ANA+ 3 768 patients was 58.92%.Among the 1 704 cases in the IIFA-ANA+/LIA-ANAs+ group,the consistent rates between single ANA fluorescence pattern and specific antibodies respectively were 82.61% in nuclear speckled pattern,82.80% in nuclear homogenous pattern,82.86% in nucleolus pattern,100% in centromere pattern,and 69.29% in cytoplasmic pattern.In the IIFA-ANA-/LIA-ANAs+ group,the corresponding fluorescent pattern to the specific antibody appeared after the dilution ratio was decreased.We conclude that the results of LIA-ANAs can not be predicated simply according to the fluorescence patterns.Clinical doctors would be able to decide whether a patient needs to undergo specific antibodies according to both the patients' condition and the IIF-ANA fluorescence patterns.
