摘要
目的观察应用中西医结合疗法治疗慢性乙型肝炎(乙肝)肝衰竭的临床疗效。方法 68例慢性乙肝肝衰竭患者随机分为2组,治疗组33例,对照组35例。对照组给予常规护肝、对症和支持治疗。治疗组在给予与对照组相同治疗的基础上第1周采用0.9%氯化钠溶液结肠透析及大黄乌梅合剂中药保留灌肠,1周后改为中药口服治疗。结果与对照组比较,治疗组总胆红素、血浆内毒素明显降低,凝血指标明显改善(P<0.05)。治疗组并发症的总发生率较对照组明显减少(P<0.05)。治疗组临床有效率为84.8%,对照组为51.4%,治疗组疗效明显优于对照组(P<0.05)。结论中西医结合治疗慢性乙肝肝衰竭可明显提高临床疗效。
Objective To investigate the therapeutic efficacy of traditional Chinese and western medicine on liver failure due to chronic hepatitis B. Methods Sixty-eight cases of liver failure due to chronic hepatitis B were randomly divided into two groups, with 33 in treatment group and 35 in control group. The control group was given combined modality therapy, including liver-protect- ing, symptomatic and supporting treatment. The treatment group, in addition to the same therapy as the control group, was given colon dialysis with 0.9% of sodium chloride solution and retention enema with the decoction of dahuang （Rheum officinale） and wumei （Prunus mume） at week 1 of treatment, and then switched to oral intake of Chinese herbal medicine. Results The total biliru- bin and endotoxin levels in the treatment group decreased more significantly than those in the control group （P〈0.05）. Blood coagulation parameters in the treatment group improved more significantly than those in the control group （P〈O.05）. The occurrence rates of complications of the treatment group were significantly lower than those of the control group （P〈0.05）. The effective rate of the treatment group was 84.8%, significantly higher than that of the control group （51.4%） （P〈0.05）. Conclusion Combined treatment of traditional Chinese and western medicine can improve the therapeutic efficacy on liver failure due to chronic hepatitis B significantly.
