摘要
目的探讨支气管动脉栓塞治疗咯血,以及相关咯血病变相同病理基础。比较不同栓塞剂的应用范围。方法入选20例咯血患者行CT和DSA检查,明确病变后施血管栓塞治疗。结果支气管扩张12例,肺结核2例,肺癌4例,支气管动脉-肺动脉瘘合并支气管扩张1例,肺隔离症1例。20例患者肺部X线平片或CT检查提示病变部位与出血动脉分布区符合。DSA造影明确病变支气管动脉,造影见支气管动脉增粗、走行迂曲等改变。栓塞后造影见末梢血管闭塞,主干近端显影。栓塞后咯血立即停止19例,1例患者咯血量明显减少,治疗后逐渐稳定。术中、术后患者无不适反应,未出现异位栓塞、脊髓损伤等严重并发症。随访24个月,无咯血复发。咯血均得到有效控制。结论靶血管栓塞对咯血的治疗效果明显,治疗方法肯定。
Objective To investigate the treatment of bronchial artery embolization for hemoptysis,and related lesions of the same pathological basis of hemoptysis.To compare the different embolic agents applications.Method Twenty cases of hemoptysis were selected to undergo CT and DSA.And patients received embolization after diagnosis was conf irmed.Results Bronchiectasis was found in 12 cases,pulmonary tuberculosis in two cases,lung cancer in 4 cases,bronchial-pulmonary artery f istula combined with bronchiectasis in 1 case,and pulmonary sequestration in 1 case.Lung X-ray or CT examinations in 20 patients showed lesions consistent with hemorrhagic arterial distribution.Bronchial artery angiography showed the lesions clearly,and thickening of bronchial artery,tortuous distribution pattern and other changes was found.Peripheral vascular angiography after embolization showed occlusion of the proximal trunk.Immediate cessation of hemoptysis after embolization occurred in 19 cases,1 patient with hemoptysis were significantly reduced after treatment gradually stabilized.Postoperative patients had no discomfort reaction,without ectopic embolism,spinal cord injury and other serious complications.After followed for 24 months,no recurrence of hemoptysis was found.Hemoptysis had been effectively controlled.Conclusion Target embolization treatment of hemoptysis has obvious eff icacy.
