摘要
目的：为了提高严重放射性溃疡的手术疗效，争取一期愈合，防止复发。方法：采取尽量彻底清创后，以血运丰富、组织量丰厚的肌皮瓣、轴型皮瓣填充修复创面。结果：１７例中，一期愈合１３例，占７６．４７％，延期愈合４例，占２３．５３％，随访３个月至６年，无复发。结论：对于严重放射性溃疡的手术治疗，应尽量彻底清创后，采用血运丰富的皮瓣或肌皮瓣修复创面，同时皮瓣或肌皮瓣的组织量要丰厚，充分填充创面，不留死腔，是争取一期愈合的关键。
Objective:To improve the operation effection of serious postradiation ulcer,strive for the heal by the first intention,prevent to have a relapseMethods:after thorough debridement as far as possible,adopt myocutaneous flap and axial pattern skin flap with both abundant blood supply and tissue,to repair and fill in the woundsResults:Among the 17 cases,13 cases were healed by first intention(7647%),healing was delayed in 4 cases(2353%)Three months or six years later,there is no relapseConclusion:For the surgical treatment of serious postradiation ulcer,we should make complete debridement as far as possible,then use flap with blood supply or myocutaneous flap to repair wounds,in the meantime,the crux to strive for the heal by the first intention is to fill in the wounds completely and prevent retaining hollow,and the amount of the flap and myocutaneous flap tissue must be rich and thick
